严重创伤患者血小板计数动态变化和预后的关系

Relationship Between the Platelet Count Dynamic Changes and Prognosis in Patients After Severe Trauma

目的 探讨严重创伤患者伤后血小板计数水平及早期动态变化和预后的关系。方法 2010年5月～2015年5月共收集严重创伤患者50例（记为观察组）、非严重创伤患者50例（记为对照组）,比较两组伤后不同时间点血小板计数水平,同时根据严重创伤患者预后情况将其分为存活组与死亡组,比较这两组患者伤后血小板计数变化,并对血小板计数与预后关系行Pearson相关分析。结果 50例严重创伤患者伤后血小板计数100×109/L以下者占30.0%。伤后24 h、48 h、72 h、第7 d血小板计数水平比较,观察组均明显低于对照组（P〈0.05）,存活组均明显高于死亡组（P〈0.05）。结论 严重创伤患者伤后血小板计数降低会明显增加患者死亡率,及时纠正患者凝血功能对其预后改善具有十分重要的意义。

Objective To investigate the relationship between the platelet count level and early dynam- ic changes and prognosis in patients after severe trauma. Methods 50 patients with severe trauma （the obser- vation group）and 50 patients with non-severe trauma（ the control group）were collected from May 2010 to May 2015. The blood platelet count levels at different time points were compared between the two groups. Meanwhile, according to the prognosis of patients with severe trauma, the patients were divided into the survival group and the death group. The post-traumatic changes of blood platelet count in the two groups were compared. Pearson correlation analysis was used to analyze the relationship between platelet count and progno- sis. Results The post-traumatic platelet count of 30% of the 50 patients with severe trauma was under 100×10^9/L. 24 h,48 h ,72 h and the 7th day after trauma,the platelet count level in the observation group was sig- nificantly lower than that in the control group （P 〈 0. 05 ）, and that in the survival group was significantly high- er than that in the death group （ P 〈 0.05 ）. Conclusion The decrease of platelet count in patients after se- vere trauma will increase the mortality. It is very important to correct the coagulation function of patients in time to improve the prognosis.
