突发性耳聋患者血小板参数治疗前后的变化 被引量:6

摘要 目的研究突发性耳聋患者血小板参数的变化及有效治疗方案。方法以2012年1月到2015年4月我院收集89例突发性耳聋患者进行研究,随机数字表法分为A、B两组,A组采取微波照射治疗配合针灸综合治疗,B组单纯应用药物治疗。于治疗前后测定两组血小板参数(包括CD62 p、CD61、v WF、PAR)与血液流变学指标(包括血浆黏度、全血高切黏度、全血低切黏度)的变化,比较两组治疗总有效率。结果两组治疗前血小板参数与血液流变学指标的差异无统计学意义(P>0.05),经治疗后两组血小板参数与血液流变学指标均降低,组内前后差异有统计学意义(P<0.05),同时A组治疗后血小板参数与血液流变学指标均低于B组,组间差异有统计学意义(P<0.05),提示A组各项指标改善幅度更大。A组治疗总有效率为88.9%(40/45),B组为70.5%(31/44),A组明显更高,差异有统计学意义(P<0.05)。结论突发性耳聋的发生会引起内耳微循环障碍以致血液流变学指标与血小板参数增大,微波照射治疗配合针灸综合治疗的应用可纠正内耳缺氧状态、改善内耳微循环,降低各项血小板参数。 Objective To study the changes of platelet parameters in patients with sudden deafness and the effective treatment. Methods 89 patients with sudden deafness treated in our hospital from January 2012 to April 2015 were studied. According to the random number table method ,the patients were divided into A, B of two groups. Group A was treated with hyperbaric oxygen therapy, while group B was treated with drug alone. Before and after the treatment,the changes of platelet parameters(CD62 p, CD61, vWF, PAR)and he- morheological indexes (plasma viscosity, whole blood high shear viscosity, whole blood low shear viscosity)in the two groups were determined. The total effective rates of treatment in the two groups were compared. Results Before the treatment, there were no significant differences in platelet parameters and hemorheological inde- xes between the two groups (P 〉 0. 05 ). After the treatment, the platelet parameters and hemorheological inde- xes of the two groups decreased. The before and after differences within group were statistically significant ( P 〈 0. 05 ). Besides, the platelet parameters and hemorheological indexes of group A after the treatment were lower than those of group B ( P 〈 0. 05 ), which indicated that the improvement in group A was more significant. The total effective rate in group A 88.9% (40/45)was significantly higher than that in group B 70.5% (31/44 ) (P 〈 0. 05 ). Conclusion The occurrence of sudden deafness can cause the inner ear mierocirculation dis- turbance and lead to the increase of hemorheological indexes and platelet parameters. The application of hyper- baric oxygen therapy can correct the inner ear anaerobic state, improve the inner ear microcirculation and re- duce platelet parameters.
