背景期、增殖期糖尿病视网膜病变凝血功能异常变化的临床研究 被引量:2

摘要 目的研究背景期及增殖期糖尿病视网膜病变患者的凝血功能异常变化。方法选择2012年3月到2015年9月来我院收治2型糖尿病患者96例作为观察组,根据视网膜病变情况分为背景期(n=54)与增殖期(n=41),另从同期来我院接受健康体检者中随机抽取96例作为对照组。分别测定三组患者血糖指标(FPG、2h PG)、凝血功能(a PTT、TT、Fbg)与血小板参数(MPV、PLT)。结果对照组FPG、2h PG均为最低,背景期及增殖期糖尿病视网膜病变患者均高于对照组,组间差异有统计学意义(P<0.05),同时增殖期FPG、2h PG均高于背景期,组间差异有统计学意义(P<0.05)。三组TT的两两比较均无统计学意义(P﹥0.05);对照组a PTT最高、Fbg最低,背景期及增殖期a PTT低于对照组、Fbg高于对照组,差异有统计学意义(P<0.05);同时增殖期Fbg高于背景期,差异有统计学意义(P<0.05)。三组PLT之间的两两比较均无统计学意义(P﹥0.05),对照组MPV最低,背景期及增殖期均高于对照组,差异有统计学意义(P<0.05),同时增殖期高于背景期,差异有统计学意义(P<0.05)。结论血液功能改变为糖尿病视网膜病变的重要指标,通过对患者凝血指标等的测定可有效发挥监测患者病情进展的作用。 Objective To study the abnormal changes of coagulation function in background and pro- liferative diabetic retinopathy. Methods 96 patients with type 2 diabetes treated in our hospital from March 2012 to September 2015 were selected as the observation group. According to the conditions of retinopathy, they were divided into background phase (n = 54)and proliferative phase (n = 41 ). In addition ,96 persons who accepted the health examination in our hospital during the same period were randomly selected as the control group. Blood glucose indexes( FPG ,2h PG), coagulation function indexes( aPTT ,TT, Fbg)and platelet param- eters ( MPV, PLT) in the three groups were determined. Results FPG and 2h PG of the control group were the lowest and those of patients with background and proliferative diabetic retinopathy were higher than those of the control group(P 〈 0. 05 ). Meanwhile, FPG and 2h PG in proliferative phase were higher than those in background phase( P 〈 0.05 ). There were no statistical significance in comparison of TT btween any two groups of the three groups ( P 〉 0. 05 ) ; aPTT of the control group was the highest and Fbg was the low- est. aPTT in background and proliferative phase were lower than those in the control group, while Fbg was higher than that in the control group ( P 〈 0. 05 ) ; Besides, Fbg in proliferative phase was higher than that in background phase( P 〈 0. 05 ). There was no statistical significance in comparison of PLT btween any two groups of the three groups (P 〉 0. 05 ). MPV of the control group was the lowest and that in background and proliferative phase were higher than those in the control group ( P 〈 0. 05 ). Meanwhile, that in proliferative phase was higher than that in background phase ( P 〈 0.05 ). Conclusion The change of blood function is an important index of diabetic retinopathy. The function of monitoring patient' s progress of disease can be effectively developed by determining coagulation indexes in patients.
