脑外伤术后再出血临床分析及防治对策

Analysis of Brain Trauma Postoperative Bleeding and Prevention Measures

目的探讨脑外伤患者术后再出血的临床情况,分析再出血原因,并提出防治对策。方法选取我院于收治的脑外伤术后再出血患者40例,对患者的病历资料进行回顾性分析。结果再出血部位:原出血灶近旁15例,原出血灶对侧或远隔部位13例,原挫伤出血灶12例。预后:恢复良好13例,轻度残疾10例,中度残疾7例,重度残疾5例,植物状态2例,死亡3例。结论脑外伤术后再出血的原因主要有术中止血不彻底、骨窗设计不合理、减压过快、麻醉复苏时血压升高等,而根据出血原因实施相应的预防对策能够有效减少再出血。

Objective To investigate the clinical situation and causesof brain trauma postoperative bleedingand propose countermeasures. Methods 40 cases of Brain trauma postoperative bleeding admitted to our hospital were analyzed retrospectively. Results Rebleeding site： near the original hemorrhage in 15 cases,the original hemorrhage contralateral or distant parts of the 13 cases,12 cases of primary contusion hemorrhage. Prognosis： good recovery in 13 cases,10 cases of mild disability,moderate disability in 7 cases,5 cases of severe disability,vegetative state in 2 cases,3 deaths. Conclusion The main reason of brain trauma postoperative bleeding is Hemostasis,bone window design is unreasonable, excessive pressure,blood pressure and other anesthesia recovery,according to the causes of bleeding implementation of appropriate preventive measures can effectively reduce rebleeding.
