清热固胎汤联合黄体酮治疗复发性流产(血热证)的临床研究

The Clinical Research of Qingre Gutai Decoction(清热固胎汤) combined with Progesterone on Habitual Abortion(Blood Heat Syndrome)

目的:探讨清热固胎汤联合黄体酮治疗血热型复发性流产的临床效果。方法:选择血热型复发性流产患者为本次研究对象,按照随机自愿的原则分为治疗组和对照组。两组在常规临床治疗的基础上,均给予黄体酮,治疗组在此基础上加服自拟清热固胎汤,以7 d为1个疗程,连续治疗4个疗程。治疗后,观察比较两组患者治疗前后症状体征变化,并检测分析治疗前后患者血清性激素、相关免疫抗体和细胞因子的含量变化。结果:治疗组治疗后愈显率为62.8%,总有效率为90.7%;对照组分别为35.3%、72.1%,治疗组优于对照组(P<0.05);治疗后,两组血清E2、P以及β-HCG含量均显著升高(P<0.05),且治疗组治疗后血清E2、P以及β-HCG含量均显著高于对照组(P<0.05);治疗后,两组血清IL-2含量均显著降低(P<0.05),而IL-4含量均显著升高(P<0.05),治疗组治疗后血清IL-2含量显著低于对照组(P<0.05),而IL-4含量显著高于对照组(P<0.05);治疗后,治疗组As Ab、Ao Ab、Em Ab及Ac Ab抗体转阴率均显著高于对照组(P<0.05)。结论:清热固胎汤联合黄体酮治疗血热型复发性流产,能够调节患者内分泌功能,抑制和改善病理性免疫反应。

Objective： To observe the clinical effect of Qingre Gutai decoction（ 清热固胎汤, QRGTD）combined with progesterone on Habitual abortion（blood heat syndrome）. Methods： The patients with habitual abortion（blood heat syndrome） were randomly divided into treatment group and control group. On the basis of routine clinical treatment in both groups, progesterone was given, and the treatment group based on it was given QRGTD with 7 days for a period of treatment. After 4 consecutive periods of treatment, the symptoms and signs of change, the serum sex hormones, related to the variation in content of immune antibody and cytokines of the two groups were analyzed and compared. Results： The markedly effective rate of the treatment group after treatment was 62.8% and the total effective rate was 90.7%, which were significantly higher than those of the control group（P 0.05）. After treatment, the serum E2, P, and β-HCG levels of the two groups significantly increased（P0.05）, and the treatment group was significantly higher than those of the control group（P0.05）.After treatment, the serum IL-2 levels of the two groups significantly decreased（P0.05）, but the content of IL-4 significantly increased（P 0.05）, the serum IL-2 of the treatment group after treatment was significantly lower than control group（P0.05）, but the content of IL-4 was significantly higher than the control group（P0.05）.After treatment, the As Ab, Ao Ab, Em Ab and Ac Ab antibody overcast rate of the treatment group were significantly higher than those of the control group（P0.05）. Conclusion： The application of QRGTD combined with progesterone can regulate the endocrine function of the patients with habitual abortion（blood heat syndrome）, restrain and improve pathological immune responses.
