非小细胞肺癌患者手术后预后与信号转导和转录激活因子3的关系

Prognostic significance of signal transducers and activators of transcription 3 and p - signal trans- ducers and activators of transcription 3 expression in patients with non - small cell lung cancer

目的探讨转录信号转导子与激活子3（STAT3）、激活的STAT3（p-STAT3）的表达与非小细胞肺癌患者手术后预后的关系。方法对接受手术治疗的72例PI—Pma期非小细胞肺癌患者进行研究，用免疫组织化学检测肺癌组织中STAT3、p-STAT的表达；Kaplan—Meier法计算复发率；Log—rank检验比较复发率；Cox回归多因素分析判定独立的危险因素。结果细胞质和（或）细胞核内出现棕黄色颗为STAT3表达阳性，细胞核内出现棕黄色颗粒为p-STAT3表达阳性。72例非小细胞肺癌（NSCLC）患者癌组织标本中，STAT3、p-STAT3表达阳性的例数分别为61例（84．7％）、39例（54．2％）。本组患者3年生存率54．2％，与其显著相关的因素有：STAT3、p-STAT3的表达、肿瘤大小、淋巴结有无转移（pN）以及pTNM分期。Cox多因素回归分析显示：淋巴结转移是NSCLC患者手术后3年生存率的独立危险因素。结论癌组织中STAT3、p-STAT3的表达、肿瘤大小、淋巴结有无转移、pTNM分期是非小细胞肺癌患者手术后3年生存率的显著相关因素，淋巴结转移是其独立危险因素。

Objective To investigate the prognostic significance of signal transducers and activa- tors of transcription 3 （ STAT3 ） and p - STAT3 in patients with non - small cell lung cancer （NSCLC）. Methods A total of 72 NSCLC patients were enrolled in this study from September 2011 to December 2011. All patients underwent complete tumor resection. We obtained 72 NSCLC specimens from these pa- tients. The immunehistochemistry was used to detect the expression levels of STAT3, and p - STAT3 in NSCLC specimens. Modeling Kaplan - Meier survival curves were used to perform univariate analysis. The survival rate was calculated by log - rank test. The Cox proportional hazard model was used to carry out the multivariate analysis. Results Cytoplasm and/or cell nucleus had the positive expression of STAT3 pro- tein, and the positive expression of p - STAT3 protein was located in cell nucleus, with the diagnostic sen- sitivity being 84. 7% （61/72）, and 54.2% （39/72） respectively. The 3 - year survival rate of the 72 pa- tients was 54. 2% , and significantly associated with STAT3 and p - STAT3 expression, pT, pN and pTNM. The pN was an independent risk factor indicated by the results of Cox regression multivariate analy- sis. Conclusion STAT3 and p - STAT3 expression, pT, pN and pTNM stage were the significantly related factors for the 3 - year survival rate in patients after operation. However, pN was the independent relevant factor.
