四逆散加减对胃食管反流性咳嗽患者诱导痰细胞及相关介质水平的影响

Effects of modified Sini powder on induced sputum cells and related medium in patients with gastroesophageal reflux cough

目的观察四逆散加减对胃食管反流性咳嗽(GERC)患者诱导痰细胞及相关介质水平的影响。方法将69例GERC患者随机分为2组。治疗组36例予四逆散加减治疗,对照组33例予奥美拉唑肠溶片治疗。2组均治疗2个月后统计临床疗效,并检测2组治疗前后诱导痰细胞及相关介质水平变化,使用莱塞斯特咳嗽生命质量问卷(LCQ)评估2组患者治疗前后咳嗽方面特定健康状况情况。结果治疗组总有效率94.44%,对照组总有效率75.76%,2组比较差异有统计意义(P<0.05),治疗组疗效优于对照组。2组治疗后诱导痰细胞中性粒细胞百分比及白细胞介素-8(IL-8)、P物质(SP)、肥大细胞类胰蛋白酶(MCT)水平下降(P<0.05),单核巨噬细胞百分比升高(P<0.05),且治疗后2组组间比较差异亦有统计学意义(P<0.05)。2组治疗后LCQ评分中生理问题、心理问题、社会问题评分及总分均较本组治疗前提高(P<0.05);治疗组治疗后生理问题、社会问题评分及总分高于对照组治疗后(P<0.05)。结论四逆散加减治疗GERC疗效确切,且能使诱导痰细胞中性粒细胞百分比及IL-8、SP、MCT水平下降,单核巨噬细胞百分比升高,改善患者生活质量。

Objective To observe the effects of modified Sini powder on induced sputum cells and related medium in patients with gastroesophageal reflux cough（ GERC）. Methods 69 GERC patients were randomly divided into two groups. 36 cases in treatment group were treated by modified Sini powder,and 33 cases in control group were treated by omeprazole tablets. The curative effect was evaluated 2 months after treatment in two groups. The levels of induced sputum cells and related medium before and after treatment were observed in two groups. The specific health condition of cough before and after treatment was evaluated by Leicester cough questionnaire（ LCQ）. Results The total effective rate in treatment group（ 94. 44%） was superior to that in control group（ 75. 76%,P 〈 0. 05）.The scores of physical status,psychological status,social issue and total score after treatment were increased in two groups as compared with those in before treatment（ P 〈 0. 05）. The scores of psychological status,social issue and total score after treatment in treatment group were higher than those in control group（ P 〈 0. 05）. The levels of NEU,IL- 8,SP and MCT after treatment were decreased in two groups（ P 〈 0. 05）,and the mononuclear macrophage was increased（ P 〈 0. 05）,and there were statistical differences on such changes between two groups（ P 〈 0. 05）. Conclusion Modified Sini powder has exact effect on the treatment of GERC,can improve the life quality of life,and decrease the levels of NEU,IL- 8,SP and MCT,increase mononuclear macrophage level.
