分娩期高龄初产妇心理健康状况及分娩结局分析

Psychological state and delivery outcomes of elderly primiparae

目的探讨分娩期高龄初产妇的心理健康状况及分娩结局方法采用90项症状清单（SCL-90）评估2013年1月至2014年6月产科收住的高龄初产妇65例（观察组）和同期住院的年轻初产妇60例（对照组）的心理健康状况。同时比较2组的分娩方式、总产程时间、产后2 h出血量及新生儿1 min Apgar评分等分娩结局。结果观察组抑郁、焦虑因子分高于对照组,差异有统计学意义（t=2.181、2.143,P〈0.05）;观察组阴道分娩率低于对照组、剖宫产率高于对照组,差异有统计学意义（χ~2=42.396,P〈0.05）。观察组总产程时间长于对照组,差异有统计学意义（t=2.421,P〈0.05）。观察组产后出血量多于对照组,差异有统计学意义（t=2.189,P〈0.05）。结论相对于年轻初产妇,高龄初产妇在分娩期存在明显的焦虑、抑郁等消极情绪,分娩结局欠佳。应加强高龄初产妇在围生期的身心监护,有利于改善分娩结局。

Objective To understand the psychological state and delivery outcomes in elderly primiparae.Methods A total of 65 elderly primipara cases as an observation group and 60 young primipara cases as a control group were investigated for their psychological state and the results were evaluated by Symptom Checklist 90（SCL-90）, and the delivery outcomes（including delivery route, total delivery time, postpartum hemorrhage and Apgar score） were also investigated and evaluated from January 2013 to June 2014. Results The scores of depression and anxiety factors in the observation group were higher than thosein the control group（t=2.181, 2.143,P〈0.05）. The rate of natural childbearing in the observation group was significantly lower than that in the control group, and the rate of caesarean in the observation group was significantly higher than that in the control group（χ2=42.396, P〈0.05）. The total delivery time in the observation group was longer than that in the control group（t=2.421, P〈0.05）. The postpartum hemorrhage amount in the observation group was more than that in the control group（t=2.189, P〈0.05）. Conclusion Compared with young primiparae, the el-der primiparaeexistin negative emotions and poor delivery outcomes. Hence, we should strengthen the monitoringof so-matopsychic conditions of elderly primiparaeduring the perinatal period in order to improve their delivery outcomes.
