无创呼吸机对急性左心衰患者治疗效果及预后的影响

Effect of noninvasive ventilator in treatment of acute left heart failure and its influence on prognosis

目的探讨无创呼吸机对急性左心衰患者治疗效果及预后的影响。方法选择我院自2014年1月到2015年1月收治的70例急性左心衰患者为研究对象,按照随机数字表法将所有患者分成观察组和对照组,对照组患者采用临床常规治疗,观察组在常规治疗基础上配合无创呼吸机辅助治疗,比较两组治疗临床疗效及动脉血气监测的变化情况。结果经治疗后,观察组Pa O2、Pa CO2、RR、HR及Sp O2指标均得到显著改善,且改善程度显著优于对照组(P<0.05);观察组总有效率为94.3%,显著高于对照组总有效率为82.9%(P<0.05)。结论急性左心衰患者采用无创呼吸机辅助治疗,能够显著改善患者的临床症状,治疗效果明显,值得向临床推广。

Objective To investigate the effect of noninvasive ventilator in the treatment of acute left heart failure and its influence on prognosis. Methods A total of 70 patients with acute left heart failure who were admitted to our hospital from January 2014 to January 2015 were enrolled as study subjects, and according to the random number table, they were divided into observation group and control group. The patients in the control group were given routine treatment, and those in the observation group were given the adjuvant therapy with noninvasive ventilator in addition to the routine treatment. The clinical outcome and changes in arterial blood gas parameters were compared between the two groups. Results After treatment, the observation group showed significantly more improvements in the parameters of PaO2, PaCO2, RR, HR, and SpO2 compared with the control group（P〈0.05）. The observation group also had a significantly higher overall response rate than the control group（94.3% vs 82.9%, P〈0.05）. Conclusion In patients with acute left heart failure, adjuvant therapy with noninvasive ventilator can significantly improve the patients＇ clinical symptoms and has a good therapeutic effect. Therefore, it holds promise for clinical application.
