东芝64排螺旋CTA在急性主动脉综合征诊断中的应用价值

Application value of Toshiba 64 row spiral CTA in diagnosis of acute aortic syndrome

目的对64排螺旋CTA在急性主动脉综合征诊断中的应用价值进行初步的研究。方法选取58例因临床怀疑急性主动脉综合征病变患者,应用东芝64排螺旋CTA进行检查,随后行主动脉三维血管图像重建,对其影像征象进行回顾性分析。结果所有病例最终均经手术或血管造影确诊,其中主动脉夹层(aortic dissection,AD)42例(Stanford A型17例,B型25例);主动脉壁间血肿(intramural aortichematoma,IMH)11例;穿透性粥样硬化性溃疡(penetratingatherosclerotic aortic ulcer,PAU)5例。结论 64排螺旋CTA作为临床影像学检查之一,具有快捷、无创的等优势,能为主动脉夹层、主动脉壁间血肿、穿透性粥样硬化性溃疡等急性主动脉综合征主要疾病提供客观的影象学征象,对该病的治疗提供直接依据,在临床诊断及鉴别诊断上也具有重要价值。

Objective To study the value of 64 row spiral CTA applying in the diagnosis of acute aortic syndrome. Methods Selected 58 patients with clinical suspected lesions with acute aortic syndrome,with the application of Toshiba 64 row spiral CTA inspection,then the image reconstruction of 3d line aortic vascular,the imaging signs were retrospectively analyzed. Results All cases finally or angiography confirmed by surgery,including 42 cases of aortic dissecting aneurysm（ Stanford type A 17 cases,25 cases of type B）; aortic intramural hematoma in 11 cases,reconstruction image to see the aortic lumen crescent or ring without reinforced high density shadow; Penetrating atherosclerotic ulcer in 5 cases. Conclusions As one of the clinical imaging examination,64 row spiral CTA has the advantages such as rapid,noninvasive,for aortic dissection,aortic intramural hematoma,penetrating atherosclerotic ulcer and acute aortic syndrome mainly provide objective imaging signs of the disease,and provide a direct basis for the treatment of the disease,it also has important value in clinical diagnosis and differential diagnosis.
