出院随访对脑卒中患者出院后遵医行为及生活质量的影响研究

Effect of discharge follow-up on compliance behavior and quality of life of patients with stroke after discharge

目的探讨出院随访对脑卒中患者出院后遵医行为及生活质量的影响,以期探求有效干预措施。方法选择自2012年2月至2015年2月我科出院的2600例脑卒中患者,按照出院顺序,分为两组,奇数者为对照组,予以常规出院医嘱,偶数者为观察组,予以出院随访,每组1300例。观察两组患者的遵医情况、采用BI量表比较两组患者的日常生活能力,采用SAS量表比较焦虑情绪改善情况。结果与对照组比较,观察组的完全遵医比例、遵医率增加明显,部分遵医及完全不遵医比例降低明显,差异具统计学意义(P<0.05)。出院前,两组患者的BI评分、SAS评分比较,差异均无统计学意义(P>0.05);随访后3个月,与对照组比较,观察组BI评分升高明显、SAS评分降低明显,差异具有统计学意义(P<0.05)。结论出院随访可提高脑卒中出院患者的遵医行为,提高生活质量,降低焦虑情绪的发生,具有较好临床推广价值。

Objective To discuss the effect of follow-up to post-discharge cerebral apoplexy patients＇ following doctor＇s instruction behaviors and life qualities,in further to explore effective intervening measures.Methods 2,600 cases cerebral apoplexy patients who left hospital from our department from Feb. 2012 to Feb.2015 were selected as research objects,depended on the order of discharge divided them into 2 groups,odd number patients as control group,they were adopted normal discharge doctor＇s advice; even number patients as observe group,they were adopted follow-up,1300 cases in each group. We observed two group patients＇ following doctor＇s instruction status,adopted BI scale to compare two group patients ＇ daily living activities,adopted SAS scale to compare anxiety improvement. Results Compared with control group,observe group patients ＇ totally following doctor ＇s instruction percentage,following doctor ＇s instruction percentage,partly following doctor ＇s instruction percentage and don＇t following doctor＇s instruction percentage obviously decreased,the differences had statistical significances（ P〈0. 05）. Before discharge,compared two group patients＇ BI scores,SAS scores,the differences all hadn＇t statistical significances（ P〈0. 05）; follow-up 3 months,compared with control group,observe group patients ＇ BI scores obviously increased,SAS scores obviously decreased,the differences had statistical significances（ P〈0. 05）. Conclusions Discharge follow-up can improve the compliance behavior,improve the quality of life,reduce the incidence of anxiety,and has better clinical value.
