金属铰链外固定联合肘外侧手术入路内固定治疗38例肘关节三联征的应用价值分析

Application of the external fixation of metal hinge combined with the internal fixation of elbow lateral surgical approach in the treatment of 38 cases of elbow joint triad

目的 :探讨金属铰链外固定联合肘外侧手术入路内固定治疗肘关节三联征的应用价值。方法 :回顾分析2012年3月—2015年9月在我院就诊治疗的肘关节三联征老年患者76例,按随机分配原则分为观察组和对照组,观察组患者给予金属铰链外固定联合肘外侧手术入路内固定治疗,对照组患者给予常规石膏外固定联合外侧手术入路治疗,对比分析两组患者术后骨折愈合、肘关节屈伸活动范围、肘关节总体疗效评分和满意度情况。结果 :观察组患者术后骨折愈合时间少于对照组,肘关节活动能力优于后者(P<0.05);观察组、对照组患者术后Mayo评估优良率和满意度分别为89.47%和95.13%和78.94%和86.65%(P<0.05)。结论 :本疗法能有效增加老年患者肘关节稳定性,有助于患者进行康复训练,加快患者肘关节功能恢复,提高患者肘关节三联征的治疗效果。

Objective： To discuss the application of the external fixation of metal hinge combined with the internal fixation of elbow lateral surgical approach in the treatment of elbow joint triad. Methods： Seventy-six cases of patients with elbow joint triad were retrospectively analyzed, who were treated from March, 2012 to September, 2015 in our hospital, and then were randomly divided into an observation group and a control group with thirty-eight cases each. The observation group received the external fixation of metal hinge combined with the internal fixation of elbow lateral surgical approach while the control group received the external fixation of conventional plaster combined with lateral surgical approach. The postoperative fracture healing time, the range of elbow motion, and the scores of overall efficacy of elbow and the satisfaction of patients were analyzed for all the patients. Results： The postoperative fracture healing time was less and the ability of elbow motion was much better in the observation group than the control group（P〈0.05）; The excellent rate of postoperative Mayo assessment and the satisfaction of patients were 89.47% and 95.13% in the observation group and 78.94% and 86.65% in the control group（P〈0.05）, respectively. Conclusion： This therapy can effectively increase the stability of the elbow of elderly patients and is helpful to the rehabilitation training and the acceleration of the recovery of elbow function of patients and can improve the therapeutic effect of patients with elbow joint triad.
