摘要
目的:分析尘肺病中矽肺和煤工尘肺的并发症状肺大泡和肺结核的X射线影像,探讨尘肺病类型与伴随症状并发率的关系。材料与方法:选择并统计出2013年1月至2015年12月在中心门诊部根据《尘肺病诊断标准》诊断为矽肺和煤工尘肺的病例为观察组共923例,及87例无尘肺病例为正常对照组,进行回顾分析。观察组中矽肺组746例,煤工尘肺组177例。统计各组在不同病变阶段所出现的肺大泡和肺结核并发症状,用SPSS17.0分析其是否具有相关性及统计学意义,观察X射线影像特点并结合分析结果进行探讨。结果:矽肺并发肺大泡患者共76例,占总矽肺病患者的10.19%;煤工尘肺并发肺大泡患者共2例,占总煤工尘肺病患者的1.13%,矽肺肺大泡并发率显著高于煤工尘肺,差异有统计学意义(χ2=17.486,P<0.05)。当矽肺发展为叁期矽肺时,肺大泡并发率为28.77%,显著高于壹期、贰期,差异有统计学意义(χ2=99.518、χ2=36.350,P<0.05);矽肺与煤工尘肺肺结核并发率以及各分期之间比较差异无统计学意义(P>0.05)。结论:矽肺患者随着疾病严重的程度的增加,并发肺大泡风险增加。矽肺相比于煤工尘肺更易并发肺大泡,两者均容易伴发肺结核。
Purpose: To investigate pneumoconiosis silicosis and coal dust lung concurrent symptoms of quantitation and tuberculosis X-ray image and with the complication rate. Materials and Methods: According to the Diagnostic Criteria of Pneumoconiosis and statistics in 2013 January to December 2015 in diagnosed cases of silicosis and coal dust lung as observation group,a total of 923 patients and 87 cases of pneumoconiosis cases as normal control group,were retrospectively analyzed. The observation group in the Silicosis group 746 cases,177 cases of coal dust lung group. Statisticed of each group in different pathological stages of the bullae of lung and pulmonary tuberculosis complicated with symptoms,using SPSS17. 0 analysis whether it has the correlation and statistical significance,observed X-ray image characteristics and combined with the results of the analysis to explore. Results: silicosis complicated with pulmonary bulla patients with a total of 76 cases,accounted for total silicosis patients 10. 19%; coal dust lung pulmonary disease complicated with pulmonary bulla patients with a total of 2 cases,accounting for 1. 13% of total coal industry and trade of lung disease,silicosis and pulmonary bullae concurrent rate was significantly higher than that in the lung of coal industry and trade,the difference is significant( χ2= 17. 486,P〈0. 05). When the development of silicosis is the third phase of silicosis and complicated with pulmonary bulla rate is 28. 77%,which was significantly higher than that in one stage,stage II,the difference has statistical significance( χ2= 99. 518,χ2=36. 350,P〈0. 05); Silicosis disease and coal dust lung pulmonary tuberculosis complicated with rate and the stage difference no statistical significance( P〈0. 05). Conclusion: Patients with silicosis disease with increasing severity,with increased risk of pulmonary bullae. Silicosis compared to coal dust lung are more likely to have concurrent pulmonary bullae,both easily associated with pulmonary tuberculosis.
