摘要
目的:探讨磁共振在膀胱癌诊断和分期中的应用价值。材料与方法:采用回顾性分析法,选取我院2014年5月-2015年8月收治的38例膀胱癌患者的临床资料。采用磁共振检测方法对38例患者进行检测,并根据病理结果将患者分为Ta-T1期、T2期、T3-T4期3组,结合术后病理分析磁共振诊断膀胱癌的敏感性。结果:经过诊断,发现38例患者中膀胱癌的患者中包含46个肿瘤,其中有2例患者有2个肿瘤,1例患者有3例肿瘤;MRI显示:38例患者中腔内型肿瘤患者28例,浸润型肿瘤8例,腔外型肿瘤1例;38例患者按照病理诊断结果分期:27例T1期,8例患者T2期,1例T3期,2例T4期;根据MRI诊断结果进行分期:26例<T1期,8例患者属于T2期,1例T3期,1例T4期。MRI诊断敏感率与术前病理诊断敏感率没有明显的区别,P>0.05,无统计学意义;术后病理分期膀胱癌病变ADC值比较,发现T1期平均ADC值明显高于T2其、T3-4期,P<0.05,有统计学意义;DWI图像上,膀胱癌显示高信号,膀胱黏膜下层显示为低信号,而膀胱肌层信号介于两者之间。结果:磁共振应用于膀胱癌诊断中具有较高的价值,对膀胱肿瘤分期的判断有较高的准确性,能为临床治疗提供科学的依据。
Purpose: To evaluate the value of MRI in bladder cancer diagnosis and staging. Materials and Methods: A retrospective analysis,our hospital in May 2014-the clinical data in August 2015 were treated 38 cases of bladder cancer patients. Using magnetic resonance detection methods in 38 patients were detected and the patients were divided according to the pathological findings of Ta-T1,T2 stage,T3-T4 of three groups; performance characteristics of MRI analysis of patients in each group and the patients with bladder cancer lesion size,urine,by the combination of magnetic resonance diagnostic sensitivity pathological analysis of bladder cancer. Results: After diagnosis,38 patients found that patients with bladder cancer contained 46 tumors,including two patients had two tumors,one patient had three cases of cancer; MRI show: 38 patients,cancer patients intraluminal 28 cases,8 cases of invasive tumors,extraluminal tumor 1 case; 38 patients according to pathological diagnosis stage: 27 cases of T1,T2 of 8 patients,1 case of T3,T4,two cases; according to MRI diagnostic results staging: 26 cases T1 period,8 cases of patients belong to T2,T3,1 case,1 case of T4. MRI and intraoperative diagnosis before pathological diagnosis rate sensitive sensitive rate no significant difference,P〈0. 05,there was not statistically significant; Without staging bladder cancer lesion ADC value comparison,we found that the average ADC value was significantly higher than T1 T2 which,T3-4 period,P〈0. 05,there was statistically significant. Conclusion: MRI is applied in the diagnosis of bladder cancer have a higher value,the judgment of the bladder tumor stage has high accuracy,can provide a scientific basis for clinical treatment.
