摘要
目的:探讨膝关节韧带损伤患者行结合BLADE技术的磁共振诊断价值,并与关节镜诊断对照分析。材料与方法:选取2015年3月-2016年3月期间18例经关节镜检查或手术证实的膝关节韧带损伤的MRI图像进行回顾性分析,对常规MRI检查中出现运动伪影的患者,结合用BLADE技术扫描。韧带损伤包括1级韧带周膜的微小损伤;2级韧带的部分撕裂损伤;3级韧带的完全撕裂损伤。结果:24条韧带损伤中,前交叉韧带损伤12条,后交叉韧带损伤2条,内侧副韧带损伤6条,外侧副韧带损伤4条。韧带损伤MRI表现:1、韧带挫伤或部分撕裂表现为韧带增粗、模糊,T2WI见局部高信号,可见韧带连续;2、韧带完全撕裂表现为明显增粗、连续性中断,断端移位,边缘模糊,信号增高。与关节镜诊断符合率为91.6%。膝关节常规扫描中出现运动伪影,结合使用BLADE技术扫描后,图像的伪影消除,韧带、半月板等清晰度明显提高;结论:MRI对膝关节韧带损伤诊断具有重要的临床应用价值,膝关节韧带损伤患者行结合BLADE技术的磁共振具有较高诊断价值,能够提高诊断准确性,为临床尽早采取有效治疗措施提供依据,从而改善患者预后,可应用于临床。
Purpose: To evaluate the diagnostic value of MRI in patients with knee ligament injury in the line binding BLADE technology,and comparative analysis with diagnostic arthroscopy. Materials and Methods: During March 2015-March 2016 in our hospital,18 cases confirmed by surgery or arthroscopic knee ligament injury were retrospectively analyzed MRI images of the patient motion artifacts appear routine MRI examination,combined with BLADE scanning technology. Ligament injuries include ligament periodontal ligament Grade 1 minute injury; Grade 2 ligament tear part damage; Grade 3 completely torn ligament damage. Results: Among the 24 ligament injury,there are 12 anterior cruciate ligament injury,2 posterior cruciate ligament injury,six medial collateral ligament injury,4 lateral collateral ligament injury. MRI showed ligament injury: 1,ligament contusion or partially torn showed ligament thickening,fuzzy,T2 WI see local high signal,visible ligament continuous; 2,completely torn ligament showed significant thickening,discontinuity shift ends bit,edge blur,increased signal intensity. And arthroscopic diagnosis rate was 91. 6%. Knee conventional scanning motion artifacts appear after BLADE technology in combination with scanning,image artifacts eliminated,ligament,meniscus and other clarity significantly improved;Conclusion: MRI has important value in evaluating the injuries of knee ligaments,The patients with knee ligament injury in the line of magnetic resonance techniques in combination with higher BLADE diagnostic value,it could improve the diagnostic accuracy for clinical treatment to take effective measures as soon as possible to provide a basis to improve the prognosis of patients,it can be used in clinical practice.
