黄嘌呤氧化酶及炎症因子在痛风与高尿酸血症中的水平变化研究 被引量:4

摘要 目的探讨黄嘌呤氧化酶及炎症因子在痛风与高尿酸血症中的水平变化情况。方法选择2010年11月~2015年11月本院收治的痛风急性发作者40例为痛风组,仅存在尿酸水平升高但未出现痛风急性发作者40例为高尿酸血症组,测定所有患者黄嘌呤氧化酶、尿酸及血沉水平,计算体内炎症因子,包括TNF—α、IL-1及hs—CRP水平,统计24h尿酸排泄量。结果痛风组患者血清黄嘌呤氧化酶及尿酸水平均显著高于高尿酸血症组,血沉快于高尿酸血症组(P〈0.01);痛风组患者体内炎症因子中TNF-α、IL-1及hs—CRP水平均显著高于高尿酸血症组(P〈0.01),痛风组24h尿酸排泄量低于高尿酸血症组(P〈0.05)。结论痛风急性发作者其血清黄嘌呤氧化酶水平显著升高,同时尿酸、TNF-α、IL-1及hs—CRP等在痛风发展过程中起到重要作用。 Objective To investigate the changes in the level of xanthine oxidase and inflammatory cytokines in patients with gout and hyperurieemia.Methods Altogether 40 cases of acute gout patients who were treated in the hospital from November 2010 to November 2015 were selected as Gout Group;while,another 40 cases of patients who only showed the symptoms of elevated uric acid levels without acute gout were selected as Hyperuricemia Group.The levels of xanthine oxidase,uric acid and erythrocyte sedimentation rate of all the patients were measured for calculation of inflammatory eytokines (including TNF-α ,iL-1 and hs-CRP levels) and statistics of the 24-hour excretion amount of uric acid,Results The serum xanthine oxidase and uric acid levels of Gout Group were significantly higher than those of Hyperuricemia Group;and the ESR was faster than that of Hyperuricemia Group (P〈0.01);the inflammatory cytokines TNF-α, IL-1 and hs-CRP levels of patients in Gout Group were significantly higher than those of Hyperuricemia Group (P〈0.01), and the 24-hour excretion amount of uric acid was lower than that of Hyperuricemia Group (P〈0.05).Conclusion Patients with acute gout show the symptoms of significantly increased serum xanthine oxidase and uric acid levels;mean- while,TNF-α, IL-1 and hs-CRP play an important role in the development of gout.
