双开门手术治疗颈椎后纵韧带骨化症的疗效分析

Analysis on the effect of double-door laminoplasty to cure ossification of posterior longitudinal ligament

目的：观察分析双开门手术治疗颈椎后纵韧带骨化症的临床疗效。方法对于2009年3月～2016年4月，共49例多节段颈椎后纵韧带骨化症患者，采用双开门或椎板切除术治疗。记录手术时间、术中出血量及术前术后JOA评分。术后给予颈托固定8～12周。随访12～36个月，术后定期行CT扫描重建记录分析融合效果。结果手术时间108.06±29.78 min，术中出血量253.32±35.13ml，术前49例患者中ASIA分级B级和C级共有16例，占32.7%，术后6个月随访8例达到D级6例达到E级。结论双开门手术治疗颈椎后纵韧带骨化症取得了满意临床效果。

Objective To observe clinical curative effect of longitudinal ligament ossification after Double-door laminoplasty of cervical vertebra. Methods A total of 49 patients with multiple posterior longitudinal ligament ossification had an operation of Double-door laminoplasty from March 2009 to April 2016. And operation time, intraoperative blood loss and preoperative JOA score were recorded. After the operation, cervical collar was used for 8 to 12 weeks. Follow-up continued 12 to 36 months. And results of CT scan and reconstruction were analyzed. Results The operation time was about 108.06＋29.78 min, the amount of blood loss was about 253.32＋35.13ml, In 49 patients there were 16 cases who belonged to ASIA grade B and grade C, accounting for 32.7.2%. When all cases were followed up for six months, there were 8 cases who were in grade D, and there were 6 cases who were in grade E. Conclusion Double-door laminoplasty can be used to treat ossification of the posterior longitudinal ligament, which can achieve satisfactory clinical effect.
