丁苯酞注射液治疗急性脑梗死CISS亚型的疗效观察

目的观察丁苯酞注射液治疗CISS亚型急性脑梗死患者的疗效。方法急性脑梗死患者339例随机分为观察Sg170例，对照组169例。每组患者按CISS分型分为大动脉粥样硬化型（LAA）、心源型（CS）、穿支动脉型（PAD）、其他原因（OE）、不明原因（UE）五个亚型。两组患者均给予基础治疗，观察组在基础治疗上，给予丁苯酞氯化钠注射液100ml（丁苯酞25mg）静脉滴注i2＆／d，疗程14d。对照组在基础治疗上，给予奥扎格雷注射液100ml静脉滴注（奥扎格雷80mg）2次／d，疗程14d。比较两组中5个亚组患者在治疗14d、90d后的NIHSS评分、mRS评分及针对脑出血等不良反应进行观察。结果两组治疗14d后，对照组和观察组患者的NIHSS、mRS评分均较入院时明显好转。观察组qLAA、PAD、OE、UE等亚组NIHSS评分下降幅度显著低于对照组（P〈0．05）。两组不良反应相当。结论应用丁苯酞注射液对急性脑梗死患者进行治疗，可取得更加良好的疗效，在不同CISS分型患者中，丁苯酞注射液对LAA、PAD、OE、UE等亚型的急性脑梗死患者改善作用显著。

Objective To investigate the clinical efficacy of butylphthalide （ NBP ） injection in patients with acute cerebral infarction according to Chinese ischemic stroke subclassification. Methods Three hundred thirty-nine cases of acute cerebral infarction were included in this study and were randomly divided into two groups （ control group and observation group ） .Each group of patients according to CISS were divided into five subtypes： LAA, CS, PAD, OE, UE.AI1 patients were given conventional treatment, the observation group patients were given NBP sodium chloride injection 100ml （ NBP 25mg ） twice daily intravenous infusion, medication 14 days. and the treatment group patients were given Ozagrel injection 100ml （ Ozagrel 80mg ） intravenously twice daily, 14 days of treatment. All patients were evaluated by NIHSS and MRs on the frst day, second weekend. And adverse reactions including intracranial hemorrhage were observed. Results Both NIHSS and MRs scores were significantly improved in two group patients after 14 days of treatment. The NIHSS scores of subgroup： LAA, PAD, OE, UE were better improved in the treatment group than in the control group. The incidences of adverse reactions were equivalent in two groups. Conclusion Butylphthalide injection treatment could achieve better results in acute cerebral infarction. In different CISS type patients, NBP injection treatment could significant improve LAA, PAD, OE, UE subtypes of acute cerebral infarction.
