摘要
目的探讨糖化白蛋白(GA)在妊娠期糖尿病(GDM)的诊断及监测中的价值。方法根据美国糖尿病协会(ADA)葡萄糖耐量试验(OGTT)作为诊断GDM标准,将398例妊娠妇女(孕期24-28周)分为正常妊娠组200例、GDMN198例,同时测定2组GA、HbAlc和空腹(FPG)、1h-2h血糖,并进行统计学分析及受试者工作特征(ROC)曲线分析。结果GDM组的GA、HbAlc和FPG、1h-2h&糖明显高于正常妊娠组(P〈0.01)。当GA诊断截点为11.85%时,ROC线下面积为0.832,敏感性为82.8%,特异性为78.5%。结论GASg提高GDM诊断的准确性,也能有效监测GDM患者短、中期血糖水平。
Objective To investigate the significance of glycafed albumin(GA) for diagnosing and monitoring of gestational diabetes mellitus(GDM). Methods A total of 398 pregnant with their estimated gestational age between 24 and 28 weeks were enrolled for GDM diagnosis using 75g oral glucose tolerance test (OGTT) based on American Diabetes Association(ADA) criteria and were classified into normal pregnancy group (200 cases) and GDM group (198 cases). GA, HbA1C, FPG, lh Glu and 2h Glu were also determined simultaneously, and the results were analyzed statistically and by receiver operating characteristic(ROC) curve. Results The GA, HbA1C, FPG, lh Glu and 2h Glu levels in GDM group were significantly higher than those in normal pregnancy group ( P〈0.01 ) .The area under ROC curve of GA was 0.832 at the cut-off value of 11.85% with sensitivity 82.8% and specificity of 78.5%. Conclusions The determination of GA can improve the accuracy of diagnosing GDM and it also can monitor of the blood glucose levels of short and medium term in patients with GDM effectively.
