摘要 目的:探讨美国ABI 9700型PCR扩增仪在肺癌患者感染白色念珠菌基因分型中的应用价值,并分析不同基因型对不同抗真菌药物的耐药情况,旨在为临床用药提供依据。方法:选取2012年5—12月肺癌患者感染的36株白色念珠菌临床分离株,采用经优化的随机扩增多态性DNA(random amplification polymorphic DNA,RAPD)方法进行分型鉴定,并对分型菌株进行药敏试验。结果:使用9700型PCR扩增仪快速、准确地将36株白色念珠菌分为A型(13株)、B型(10株)、C型(7株)、D型(6株);36株白色念珠菌对氟康唑、5-氟胞嘧啶、伊曲康唑、咪康唑、克霉唑、酮康唑的耐药株数分别为2、1、4、2、0、6株。结论:美国ABI 9700型PCR扩增仪能够对肺癌患者感染的白色念珠菌快速、准确地做出分型,从而为临床用药提供依据,是一台可靠、经济、高效的检测仪器。 Objective To describe the application value of ABI 9700 PCR amplification in genotyping of Candida albicans of lung cancer patients, and to analyze antibiotic resistance between different C. albicans genotypes. Methods From May to December 2012, 36 clinical isolates of C. albicans strains were used in the optimization of randomly amplified polymorphic DNA(RAPD) genotyping, and antibiotic sensitivity test was carried out. Results ABI 9700 PCR amplification divided 36 C.albicans strains into 3 genotypes, with 13 strains as type A, 10 strains as type B, 7 strains as type C and 6 strains as type D.There were 2 C. albicans strains resistant to fluconazole or miconazole, 4 strains resistant to itraconazole, 1 strain resistant to 5-flucytosine and 6 strains resistant to ketoconazole. No strains were resistant to clotrimazole. Conclusion ABI 9700 PCR instrument is able to perform genotyping C. albicans rapidity and accurately to provide a helpful method for clinical diagnosis of C. albicans infection in lung cancer.
