儿童患者使用万古霉素专项点评及分析

Review and Analysis of Vancomycin Usage in Pediatric Patients

目的评价万古霉素在儿童患者中的应用情况,促进其安全、有效、合理使用。方法汇总医院2014年第1季度患儿病历,抽取其中78例应用万古霉素的病例,参照《北京市医疗机构处方专项点评指南(试行)》《抗菌药物临床应用指导原则》《万古霉素临床应用中国专家共识(2011版)》对其进行调查汇总,点评其用药适应证、用法用量、处方流程、用药监测等。结果万古霉素在适应证、溶剂选用、药物相互作用等方面基本合理,不合理主要表现在给药频次、血药浓度监测、给药浓度等。结论应加强此类药物用药点评与监护,促进其合理用药。

Objective To evaluate the application of vancomycin and to promote the safe, effective and rational use of vancomycin. Methods The medical records during the first quarter of 2014 of pediatric patients were summarized. 78 cases were who had used vancomycin were randomly selected to review and analyze the drug indications, usage, dosage, prescription management, therapeutic drug monitoring （TDM） etc, according to The Guidance of Special Comments on Prescription in Beijing Medical Institutions （Trial） and The Guiding Principle of Clinical Use of Antibacterials and The Chinese Expert Consensus for Clinical Application of Vancomycin（2011 Edition）. Results The use of vancomycin was basically reasonable in indications, dissolvent selection and interactions, etc.,and the main irrational use was reflected in medication frequency, TDM, drug concentration, etc. Conclusion Pharmacists should strengthen review and pharmaceutical care in order to promote the rational use of such drugs.
