论中风后家庭访视的必要性与操作的可行性

目的讨论中风患者出院后存在中风后遗症及康复的问题,开展中风后病人出院家访,消除和减少健康风险因素的影响,提高中风患者的生活质量,减少患者医疗费用,为医院自身发展创造良好的社会环境。方法对开展中风后家庭访视前2年2362例中风患者复诊情况与开展家庭访视后2年内2292例中风患者复诊情况进行分析,论述中风后家庭访视的必要性与操作的可行性。结果开展中风后家庭访视使患者的复诊率显著下降(P<0.01),中风患者和家属的满意度明显提高(满意率>86%)。结论家庭访视解决了中风患者出院后存在的实际问题,给医院带来了很好的口碑,也为医院带来较大的经济效益。医疗护理模式由医疗逐渐扩展到社区,是目前的医疗护理的方向。但家庭访视还需要建立完善的体制,为中风患者开展家庭访视的深度、广度有待于广大医护工作者进一步实践提高。

Objective To discuss stroke sequelae of stroke patients after discharge and rehabilitation problems , stroke patients discharged after conduct home visits to eliminate and reduce the impact of health risk factors for improving the quality of life of stroke patients and reducing patient care costs , and to create a good social environment for the development of the hospital .Method After the stroke of the former to carry out home visits two years 2362 cases of stroke patients after referral cases and carry out home visits in 2 years 2292 cases of stroke referral of patients were analyzed , the feasibility of necessity after stroke home visits and operations were discussed .Result Conduct home visits after stroke patients make the referral rate was significantly decreased （ P〈0.01 ） , and satisfaction of stroke patients and their families was significantly improved （ satisfaction rate is greater than 86%） .Conclusion Family visits solved the practical problems of stroke patients after discharge , brought a good reputation for hospital , and also brought greater economic benefits for the hospital .Medical care model by the medical gradually extended to the community , it is the direction of health care .But home visits also need to establish a sound system for stroke patients conducted home visits depth and breadth , and the majority of health care workers need to be further improving practice .
