摘要
目的 探讨普米克令舒联合可必特雾化吸入对小儿喘息型肺炎临床症状及血清中相关炎性因子水平的影响。方法 将76例喘息型肺炎患儿按照治疗方法分为对照组(n=37)与观察组(n=39),两组患儿均行平喘、持续低流量吸氧以及抗生素控制感染,并给予可必特等常规治疗,观察组在此基础上给予雾化吸入普米克令舒联合治疗。比较两组患儿临床疗效、临床症状及体征消失时间、治疗前后血清相关炎性因子水平变化情况。结果 (1)观察组临床总有效率为97.44%(38/39),对照组为72.97%(27/37),差异具有统计学意义(P〈0.05);(2)观察组患儿临床症状及体征(气喘、咳嗽、肺部啰音及喘鸣音)消失时间均显著少于对照组(P〈0.05-0.01);(3)两组患儿治疗后血清IL-4、IL-6、IL-8水平均显著低于治疗前(P〈0.05-0.01),两组患儿治疗后血清IFN-γ、IL-10水平均显著低于治疗前(P〈0.05),且治疗后上述血清炎性因子水平差异均具有统计学意义(P〈0.05)。结论 普米克令舒联合可必特雾化吸入可有效改善小儿喘息型肺炎临床症状及体征,其作用机制可能是通过改变血清相关炎性因子水平来提高疗效。
Objective To investigate the effect of Pulmicort combined with Combivent atomization inhalation on clinical symptoms and serum level of inflammatory related factors in children with wheezing pneumonia .Methods 76 children with wheezing pneumonia were divided into the control group (n=37) and the observation group (n=39).All children were given asthma relieve , continuous low flow oxygen therapy , Combivent atomization inhalation , and antibiotics to control infection , and the observation group was additionally given Pulmicort .The clinical curative effect, clinical symptoms and signs disappeared time , and the change of serum inflammatory factors were compared . Results (1) The clinical total effective rate of the observation group was 97.44%(38/39), and the control group was 72.97%(27/37) (P〈0.05).(2) The clinical symptoms and signs (asthma, cough, lung sounds and wheez-ing) disappeared time was significantly shorter in the observation group than in the control group (P〈0.05-0.01). (3) After treatment, the serum levels of IL-4, IL-6 and IL-8 were significantly lower than before ( P 〈0.05 -0.01), and the serum levels of IFN-γand IL-10 were significantly lower than before (P〈0.05), and the serum lev-els of inflammatory factors showed statistically significant difference between the two groups (P〈0.05).Conclusion Pulmicort combined with Combivent atomization inhalation can effectively improve the pediatric breathing atomiza -tion inhalation pneumonia clinical symptoms and signs , and its mechanism may be through the change of serum levels of inflammatory factors associated to improve curative effect .
