糖尿病患者部分生化指标与慢性并发症的相关性分析 被引量:4

摘要 目的分析2型糖尿病患者部分生化指标与慢性并发症的相关性,为临床预防2型糖尿病患者慢性并发症提供参考。方法选取在我院就诊的1102例2型糖尿病患者为研究对象,收集患者入院时体重指数(BMI)、甘油三脂(TG)、低密度脂蛋白(LDL—C)、高密度脂蛋白(HDL-C)、糖化血红蛋白(HbAIc)UA及慢性并发症情况,进行相关性统计分析。结果校正已知混杂因素后,糖尿病大血管病变与HbAlc(OR=2.940,95%CI:1.309。3.077)、LDL-C(oR=0.568,95%C1:0.366~0.8831、BMI(OR:0.402。95%C/:0.276~0.768)密切相关;糖尿病肾病与TG(OR=1.127,95%C/:1.039~1.223)、HbAlcfOR=0.994,95%叫:O.896~0.966)密切相关:糖尿病视网膜病变与HbAlc(OR=1.591,95%C/:1.226—2.066)密切相关:糖尿病周围神经病变与HbAIc(OR=1.138.95%C/:1.011~1.280)密切相关;糖尿病足与TG(OR=1.011,95%(3:1.002~1.020)、HDL(OR=0.362,95%(21:0.204—0.642)密切相关:而糖尿病自主神经病变与生化指标相关性不大。结论不同糖尿病慢性并发症与生化指标的异常存在一定关联。应加强对糖尿病患者生化指标的监测,以及早发现和处理并发症的征象,防止或延缓严重并发症的发生。 Objective To analyze the correlation between partial biochemical indexes and chronic complications in type 2 diabetics to provide reference for clinical prevention of chronic complications in type 2 diabetics. Methods A total of 1,102 type 2 diabetics admitted to our hospital were selected in order to collect their body mass index (BMI), triglyceride (FG), low density lipoprotein(LDL-C), high density lipoprotein(HDL-C), glycosylated hemoglobin(HbAlc) and chronic complications tor statistical analysis of such correlation, Results When known confounding factors were corrected, diabetic great vascular complications were closely related to HbAlc (OR=2.940,95% CI:1.309-3.077), LDL-C (0R=0.568,95% CI:0.366-0.883) and BMI (0R=0.402,95% C1:0.276-0.768); diabetic nephropathy to TG (OR=1.127,95% C1:1.039-1.223) and HbAlc (0R=0.994,95% C1i0.896-0.966); diabetic retinopathy to HbAlc (OR=1.591,95% CI: 1.226-2.066); diabetic peripheral neuropathy to HbAlc(OR=I. 138,95% CI: 1.011-1.280); diabetic toot to TG (OR=1.011,95% CI:1.002-1.020) and HDL (OR=0.362,95% CI:0.204-0.642); however, diabetic autonomic neuropathy was not closely related to biochemical indexes. Conclusion Various diabetic chronic complications are related to abnormal biochemical indexes to some degree. Therefore, the monitoring of the biochemical indexes of diabetics should be strengthened to timely find out and handle the signs of complications in order to further prevent or delay the occurrence of severe complications.
