脉搏指示连续心排量监测在急性肾损伤容量过负荷患者血液滤过中的应用

摘要 目的探讨脉搏指示连续心排量(Pi CCO)监测在急性肾损伤(AKI)合并容量过负荷患者血液滤过中的应用。方法选择23例AKI合并容量过负荷且需要连续性静脉-静脉血液滤过(CVVH)及机械通气的患者,随机分为常规组12例和Pi CCO组11例。比较两组患者的一般资料及CVVH治疗时间、液体达平衡时间、机械通气时间、住ICU时间等指标。结果两组患者的一般资料,差异无统计学意义(P>0.05)。治疗后Pi CCO组与常规组相比CVVH治疗时间、液体达平衡时间、机械通气时间、住ICU时间均缩短(P<0.05)。结论Pi CCO监测在AKI合并容量过负荷患者的CVVH治疗中可以有效指导液体管理。 Objective To investigate the value of pulse indicator continous cardiac output (PiCCO) in hemofiltration in patients with acute kidney injury (AKI) and volume overload. Methods 23 cases of patients with acute kidney injury (AKI) and volume overload were randomly divided into control group (12 cases) and PiCCO group (11 cases). General date, the duration of CVVH, duration of achieving lfuid balance, duration of mechanical ventilation and duration of ICU stay were compared between two groups. Results The difference was not statistically signiifcant about general information between two groups (P〉0.05). Compared with the conventional group, the CVVH treatment time, the liquid reached the balance time, the mechanical ventilation time and the ICU time in the treatment group were shorter (P 〈 0.05). Conclusion PiCCO can improve the capacity management in CVVH in patients with AKI and volume overload.
