老年冠心病合并肾功能不全患者的药学服务

Pharmaceutical Care in Elderly Patients With Coronary Heart Disease Complicated With Renal Insufficiency

摘要 目的探索临床药师对老年冠心病合并肾功能不全患者的药学监护方案。方法通过与临床医师共同制定治疗方案,对患者住院期间的用药进行具体化药学服务和制定个体化监护计划。结果临床药师对血运重建的适应证,冠脉造影的注意事项、二级预防的合理用药等方面都有了深刻全面的认识。结论临床药师积极参与临床的治疗过程,确保用药安全有效,提高了整体治疗水平。 Objective To explore the clinical pharmacists in elderly patients with coronary heart disease complicated with renal insufifciency patients with pharmaceutical care program. Methods To make a treatment plan with the clinicians, to make speciifc pharmaceutical care for patients in hospital and to make individual care plan. Results Clinical pharmacists have a deep and comprehensive understanding of the indications for revascularization, the attention of coronary angiography, the rational use of the two levels of prevention, and so on. Conclusion Clinical pharmacists actively participate in clinical treatment process, to ensure safe and effective drug use, improve the overall level of treatment.
