两种估算新生儿气管内导管插入深度方法的比较 被引量:1

摘要 目的探讨更准确的估算新生儿气管内导管捅入深度的方法。方法将2011年1月至2014年12月在扬州大学医学院附属盐城市妇幼保健院住院,并行气管插管机械通气的新生儿240例,采用信封法随机按照体重或胎龄估算气管内导管的插入深度,分别作为体重组和胎龄组。根据出生体重和胎龄,将240例新生儿分为小于胎龄儿40例(2组各20例)、适于胎龄儿160例(2组各80例)以及大于胎龄儿40例(2组各20例)。根据插管后床旁x射线摄片的结果,比较2种估算插入深度方法的准确率及相关预后指标(包括气管内导管位置过深或过浅,机械通气持续时间,生后28d仍需使用氧气,以及出院前死亡等)。分别采用采用t检验或z。检验分析数据资料。结果小于胎龄儿中,胎龄组的估算准确率高于体重组[85.0%(17/20)与55.0%(11/20),x^2=4.286,P=0.038];大于胎龄儿中,体重组准确率高于胎龄组[90.0%(18/20)与60.0%(12/20),x^2=4.800,P=0.028];适于胎龄儿中,2组准确率差异无统计学意义(P〉0.05)。小于胎龄儿中,体重组气管内导管估算位置易较浅[30.0%(6/20)与5.0%(1/20),x^2=4.329,P=0.037];大于胎龄儿中,胎龄组导管估算位置易较浅[35.0%(7/20)与5,0%(1/20),x^2=5.625,P=0.018]。适于胎龄儿中,2组导管估计位置基本相当。2组患儿其他相关预后指标差异均无统计学意义(P值均〉0.05)。结论对于小于胎龄儿,采用胎龄估算气管内导管插入深度准确率较高,如按体重估算,则易出现插入深度过浅;对于大于胎龄儿,采用体重估算的准确率较高,如按胎龄估算,则易出现插入深度过浅。
