免疫标志物CD117、CD34、DOG1在胃肠道间质瘤诊断中的价值

The value of CD117,CD34 and DOG1 in the diagnosis of gastrointestinal stromal tumor

目的探讨应用Logistic回归和ROC曲线综合评价CD117、CD34、DOG1在胃肠道间质瘤(GIST)诊断中的价值。方法用CD117、CD34及DOG1抗体,采用S-P法对252例胃肠道间质瘤患者及89例胃肠病患者(包括平滑肌瘤、神经鞘瘤、纤维瘤)进行检测,通过ROC曲线分析CD117、CD34、DOG1及免疫标志物联合检测的Logistic回归结果的ROC曲线值(AUC)。结果胃肠道间质瘤患者的CD117、CD34及DOG1的阳性率均显著高于胃肠病组,在胃肠道间质瘤-胃肠病组的ROC曲线中,DOG1(0.983)>CD117(0.931)>CD34(0.922);三种免疫标志物联合检测的AUC值高于三种免疫标志物单一检测的AUC。结论联合检测CD117、CD34、DOG1对胃肠道间质瘤具有较高的诊断价值,可提高胃肠道间质瘤的诊断率。

Objective To probe into the diagnostic value of CD117,CD34 and DOG1 for GIST bylogistic regression and ROC curve. Methods The expression of CD117,CD34 and DOG1 in samples from 252 cases of gastrointestinal stromal tumor patients and 89 cases of gastrointestinal disease（ including leiomyoma,gastric schwannoma and fibroma） was detected by S- P method. By using ROC curve to analyze the ROC curve value（ AUC） of CD117,CD34,DOG1 and immune markers,respectively. Results The positive rates of CD117,CD34 and DOG1 of tumor patients in gastrointestinal stromal tumor group were significantly higher than those of gastrointestinal disease group. In gastrointestinal stromal tumors- gastrointestinal disease group ROC curve,the AUC of DOG1 was larger than the AUC of CD177,and the AUC of CD177 was larger than the AUC of CD34. The AUC of combination of CD117,CD34 and DOG1 was larger than single AUC of CD117,CD34 and DOG1. Conclusions The combined detection of CD117,CD34 and DOG1 has higher diagnostic value in GIST and it can improve the diagnosis rate of GIST.
