消膜止血调整周期法治疗子宫内膜简单型增生过长崩漏的临床观察

Clinical Observation of Treatment to Functional Metrorrhapgia of Simple Hyperplasia of Endometrium by Adjusting Cycle with Ablation and Hemostasis

[目的]观察消膜止血调整周期法治疗子宫内膜简单型增生过长崩漏的临床疗效。[方法]将80例子宫内膜简单型增生过长崩漏患者,随机分为中药组和西药组,各40例。中药组采用消膜止血调整周期法治疗,出血期服化瘀消膜方,血止后予调周Ⅰ方服用7剂,续服调周Ⅱ方10～14剂;西药组在后半周期口服安宫黄体酮片10mg,1次/日,连用14天。两组均治疗3个月。观察两组治疗前后每月阴道流血时间、流血量、子宫内膜厚度、血红蛋白,统计临床疗效,对痊愈病例治疗完成后6月进行随访。[结果]中药组临床有效率95.0%,西药组临床有效率92.5%,两组治疗后每月阴道流血时间、流血量、子宫内膜厚度、血红蛋白较治疗前均显著改善,两组间差异无统计学意义（P〉0.05）。治疗完成后痊愈病例6月后随访,中药组月经正常例数显著高于西药组,两组间差异有统计学意义（P〈0.05）。[结论]消膜止血调整周期法治疗子宫内膜简单型增生过长崩漏疗效肯定,其远期疗效优于安宫黄体酮,值得临床推广应用。

[Objective] This paper is to observe clinical efficacy on treatments to functional metrorrhapgia of simple hyperplasia of endometrium by adjusting cycle with ablation and hemostasis.[Methods] There were 80 patients with functional metrorrhagia of simple hyperplasia of endometrium in this study. They were randomly divided into two groups： 40 patients in Traditional Chinese Medicine（TCM） group and 40 patients in western medicine group. In TCM group,patients were given oral decoctions with methods of adjusting cycle with ablation and hemostasis： Hua Yu Xiao Mo Fang（HYXMF） during blooding, Tiao Zhou Fang I（TZF I） during hemostasis, Tiao Zhou Fang II（TZF II） after TZF I. In western medicine group, patients were given oral dosage of 10 mg of medroxyprogesterone acetate tablets per day over 14 days after ovulation. Treatments lasted for 3 months in both groups.Then the time of vaginal bleeding,amount of bleeding, endometrial thickness and haemoglobin level of the two groups were compared before and after treatment. Clinical effective rates were calculated, follow-ups were continued among cured patients by 6 months after treatments. [Results] Clinical effective rates reached 95% in TCM group and92.5% in western medicine group. There were obvious improvements after treatments on aspects of time of vaginal bleeding, amount of bleeding, endometrial thickness and haemoglobin level. There was no statistic difference between these two groups（ P〉0.05）. Follow-ups were continued among cured patients by 6months after treatments. Cases of patients with normal menstruation were significantly higher in TCM group than the one in western medicine group. There was statistic difference between two groups（P〈0.05）.[Conclusion]Treatment to functional metrorrhapgia of simple hyperplasia of endometrium by adjusting cycle with ablation and hemostasis has definite effect, its long-term curative effect is better than Provera, worthy clinical promotion of use.
