摘要
目的 :观察新生儿甲状腺功能低下症(congenital hypothyroidism,CH)的超声表现,探讨其与甲状腺相关激素水平的关系。方法:对100例正常对照组及154例确诊为CH的新生儿行甲状腺超声检查,并分析其影像资料。结果:154例中84例(54.55%)甲状腺明显肿大、血流信号明显增加;34例(22.08%)甲状腺基本正常;24例(15.58%)甲状腺异位或缺如;12例(7.79%)甲状腺发育不全。血清甲状腺TSH、FT_4、FT_3值在不同形态甲状腺间差异均有统计学意义(均P<0.05);CH(甲状腺异位或缺如除外)患儿甲状腺体积与TSH、FT3不相关(P>0.05),与FT4水平呈负相关(r=-0.361,P=0.004)。结论:甲状腺超声在寻找CH病因、选择合理治疗方案和CH筛查方面具有一定的临床价值。
Objective:To observe the ultrasonic diagnosis of neonatal congenital hypothyroidism(CH),and to explore its relationship with thyroid-related hormone. Methods:100 cases of normal control group and 154 cases diagnosed as CH were examined by ultrasound,and the data were analyzed. Results:There were 84 cases(54.55%,84/154) with thyroid glands diffusely enlarged and blood flow increasing significantly,34 cases(22.08%,34/154) with normal thyroid glands,24 cases(15.58%,34/154) with ectopic or athyrotic thyroid glands,and 12 cases(7.79%,12/154)with thyroid dysplasia. There was significant difference of 154 CH infants between the thyroid morphological categories and thyroid hormones(TSH,FT4,FT3,P〈0.05). There was no difference between thyroid volume and TSH or FT3(P〉0.05),while negative correlation between thyroid volume and FT4(r=-0.361,P=0.004),except for the ectopic or athyrotic thyroid group. Conclusion:Thyroid ultrasound examination has a certain clinical value for finding the cause,choosing the treatment options and neonatal screening for CH.
