醋酸亮丙瑞林微球联合体外受精-胚胎移植治疗子宫腺肌病合并不孕症的疗效观察

Observation on Efficacy of Leuprolide Acetate Microspheres Combined with IVF-ET in Treatment of Adenomyosis Complicated with Infertility

目的:探讨醋酸亮丙瑞林微球联合体外受精-胚胎移植(in vitro fertilization and embryo transfer,IVF-ET)治疗子宫腺肌病合并不孕症的临床疗效。方法:选择2014年1月—2015年1月收治的120例子宫腺肌病合并不孕症患者,按随机数字表法分为观察组和对照组,每组各60例。观察组患者给予醋酸亮丙瑞林微球排卵联合IVF-ET治疗,对照组单独进行IVF-ET治疗,观察并记录2组患者的疗效。结果:治疗后,观察组患者的子宫体积明显小于治疗前,且明显小于对照组,差异均有统计学意义(P<0.05)。观察组患者促性腺激素平均使用时间明显长于对照组,平均使用量明显高于对照组,差异均有统计学意义(P<0.05);观察组患者获卵数和优质胚胎数明显少于对照组,差异有统计学意义(P<0.05);2组患者受精率的差异无统计学意义(P>0.05);2组患者临床结局的差异无统计学意义(P>0.05)。2组患者中,妊娠成功者的受精卵、双源核以及优质胚胎率均高于妊娠失败者,差异有统计学意义(P<0.05),但2组的差异无统计学意义(P>0.05)。结论:醋酸亮丙瑞林微球联合IVF-ET治疗子宫肌腺病合并不孕症,可显著提升助孕效果,临床优势显著,值得推广。

OBJECTIVE： To investigate the clinical leuprolide acetate microspheres combined with IVF-ET in treatment of adenomyosis complicated with infertility. METHODS： 120 patients with adenomyosis complicated with infertility admitted from Jan. 2014 to Jan. 2015 were selected to be divided into observation group and control group via the random number table,with 60 cases in each. The observation group were treated with leuprolide acetate microspheres combined with in vitro fertilization and embryo transfer（ IVF-ET）,while the control group only received IVF-ET,the efficacy of two groups were observed and recorded. RESULTS： After treatment,the uterine volume of observation group were significantly less than that before treatment,and significantly less than the control group,the difference was statistically significant（ P〈0. 05）; and patients in the observation group in the Gn application time was significantly longer than that in the control group,the average application amount in the observation group was higher than that in the control group,the differences were statistically significant（ P〈0. 05）; patients in the observation group received number of oocytes and the number of high-quality embryos were significantly less than those in the control group,the difference was statistically significant（ P〈0. 05）; there was no significant difference in the fertilization rate and clinical outcomes of two groups（ P〉0. 05）. Of the two group of patients,the rate of fertilized eggs,dual nuclear rate and high-quality embryo rate in pregnancy success patients were higher than those of pregnancy failure patients,with significant difference（ P〈0. 05）,but the difference between the two groups was no statistical significance（ P〈0. 05）. CONCLUSIONS：Leuprolide acetate microspheres combined with IVF-ET in treatment of adenomyosis complicated with infertility can significantly improve the assisted reproductive effects with clinical significant advantages. It is worthy to be popularized.
