Analysis on Application of Antibiotics in 4 000 Emergency Prescriptions

目的:了解中山市人民医院(以下简称"我院")急诊抗菌药物的使用情况,为合理使用抗菌药物提供参考。方法:随机抽取我院2015年1—10月急诊处方4 000张,统计抗菌药物使用率、给药途径、抗菌药物的联合应用、各类抗菌药物的使用情况等,并对不合理用药进行分析。结果:4 000张急诊处方中,抗菌药物使用率为34.50%(1 380张/4 000张);以静脉滴注给药和单一用药为主,各占抗菌药物处方数的83.19%(1 148张/1 380张)和82.03%(1 132张/1 380张);涉及11类抗菌药物,使用率较高的为头孢菌素类、林可霉素类和氟喹诺酮类,分别为51.57%(852例次/1 652例次、19.13%(316例次/1 652例次)和9.20%(152例次/1 652例次);抗菌药物不合理使用率为18.84%(260张/1 380张)。结论:我院急诊抗菌药物的使用基本合理,但也存在部分不合理用药情况,临床医师需加强对抗菌药物合理应用知识的学习,药师应严格审方、及时反馈,共同促进合理用药,防止抗菌药物的滥用。

OBJECTIVE： To investigate the application status of antibiotics in emergency department in Zhongshan People＇s Hospital（ hereinafter referred to as ＂our hospital＂）,so as to provide reference for the rational application of antibiotics. METHODS： 4 000 emergency prescriptions in our hospital from Jan. to Oct. 2015 were randomly selected,the application data were statistically collected in terms of application rate of antibiotics, route of administration,drug combination of antibiotics and application status of different kinds of antibiotics,etc.. And the irrational drug use was analyzed. RESULTS： Of the 4 000 prescriptions,the application rate of antibiotics was34. 50%（ 1 380 /4 000 prescriptions）; and intravenous infusion and single administration took the lead,respectively accounting for 83. 19%（ 1 148 /1 380 prescriptions） and 82. 03%（ 1 132 /1 380 prescriptions）. 11 categories of antibiotics were involved, the top three drugs were cephalosporins, lincomycins and quinolones, respectively accounting for 51. 57%（ 852 /1 652 cases）,19. 13%（ 316 /1 652 cases） and 9. 20%（ 152 /1 652 cases）. The irrational rate of antibiotics was 18. 84%（ 260 /1 380 prescriptions）. CONCLUSIONS： The application of emergency antibiotics in our hospital is basically rational,but there are also some irrational phenomenon. Therefore,the clinicians need to strengthen the study of the rational application of antibiotics,and the pharmacists should strictly check and timely feedback the irrational prescriptions,so as to jointly promote the rational application of drugs and prevent the abusing of antibiotics.
