卡前列甲酯栓联合缩宫素预防中孕引产产后出血疗效分析

Effects of oxytocin combined with carboprost methylate suppository on the prevention of hemorrhage after induced labor in mid-pregnancy

目的探讨卡前列甲酯栓联合缩宫素预防中孕引产产后出血的疗效及安全性。方法选择2012年12月至2014年12月期间我院进行中孕引产的128例孕妇为研究对象,随机分为治疗组和对照组各64例,两组均在胎儿娩出后向子宫注射缩宫素20IU,治疗组在胎儿娩出后舌下含服卡前列甲酯栓2mg。比较两组患者的完全流产率、第三产程时间、引产后出血量、血压、脉搏及不良反应发生率。结果治疗组的完全流产率高于对照组,第三产程时间、引产后2h出血量及引产后24h出血量低于对照组,差异有统计学意义(P<0.05)。两组患者治疗前和产后2h的血压和脉搏的差异无统计学意义(P>0.05)。治疗组的不良反应发生率高于对照组,差异有统计学意义(P<0.05)。结论卡前列甲酯栓联合缩宫素用于中孕引产,可以减少产后出血和第三产程时间,提高完全流产率。

Objective To investigate the effects of oxytocin combined with carboprost methylatesuppository on the prevention of hemorrhage after induced labor in mid-pregnancy. Methods Atotal of 128 mid-pregnancy women undergoing induced labor in our hospital from December 2012to December 2014 were chosen,and they were randomly divided into treatment group and controlgroup with 64 cases in each group. They were all given intrauterine injection of 20 IU oxytocinimmediately after delivery, the treatment group were orally administered 2mg carbeprost immediatelyafter delivery. The complete abortion rate, the duration of third stage of labor, volumes ofpostpartum hemorrhage, blood pressure, pulse, and incidence of adverse reaction in two groupswere compared. Results The complete abortion rate in treatment group were higher than thecontrol group, the duration of third stage of labor and volumes of postpartum hemorrhage （2hours and 24 hours after delivery） in treatment group were lower than the control group, thedifference was statistically significant （ P 〈C 〇■ 05）. The differences of blood pressure and pulse（before and 2 hours after delivery） in the two groups were no significant （P〉0. 05）. The incidenceof adverse reaction in treatment group were higher than the control group, the differencewas statistically significant（P 〈0. 05）. Conclusion Application of oxytocin combined with carboprostmethylate suppository in mid-pregnancy, may reduce volume of postpartum hemorrhage,shorten duration of third stage of labor and increase the complete abortion rate.
