胃癌不同病理分型、分化程度的MRI表现

MRI features of gastric cancer with different pathological types and differentiation degrees

目的探讨不同病理分型、分化程度胃癌的MRI表现。方法收集86例经病理证实的胃癌MRI资料,观察病灶的信号特点及强化方式,与病理分型及分化程度对照分析。结果胃癌在T2WI以等信号(65.12%)为主,2例粘液腺癌均为等高信号,表现为等低信号的均是中分化或低分化。DWI胃癌以高(63.95%)或稍高(34.88%)信号为主,低分化胃癌有更高比例表现为高信号(70.83%),且出现三明治征更多(43.75%)。胃癌的强化方式以渐进性强化(62.79%)和持续性强化(34.88%)为主,低分化胃癌中持续性强化的比例相对较高(41.67%)。结论不同病理分型、分化程度的胃癌在MRI的表现具有一定特征性。

Objective To investigate MRI features of gastric cancer with different pathological types and differentiation de- grees. Methods MRI informations of 86 patients were collected, which was pathologically proved as gastric cancer. The signal intensity and enhancement patterns were analyzed and compared with different pathological types and diiferentiation degrees. Re- sults Majority of gastric cancers showed equal signal （65.12%） on T2-weighted imaging （T2 WI）. Mucinous adenocarcinomas （2 cases） showed equal-high signal on T2WI. All the lesions, which showed equal-low signal on T2WI, were moderately or poorly differentiated. On diffusion-weighted imaging （DWI）, most of gastric cancers showed high signal （63.95%） or slightly high signal （34.88%）. Poorly differentiated gastric cancers had a higher proportion with high signal （70.83%） and sandwich sign （34. 88% ）. Conclusion There were some special features of gastric cancers with different pathological types and differentiation degrees on MRI.
