摘要 目的探讨MRI诊断肩袖损伤的临床价值。方法回顾性分析2014年6月~2015年12月68例临床诊断为肩关节疾病的患者,将MRI表现与关节镜检查所见进行对比。结果68例患者中,关节镜检查证实50例为肩袖损伤,18例肩袖未见明显异常。50例肩袖损伤中,术前MRI阳性者44例,阴性者6例,敏感度为88.0%。18例关节镜未见肩袖损伤者,术前MRI阳性3例,阴性15例,特异度83.3%,诊断准确性86.8%。结论MRI在诊断肩袖损伤上具有较高的临床诊断价值,作为一种无创检查技术,值得临床推广应用。但存在一定漏、误诊情况,必要时建议辅以关节镜检查。 Objective To investigate the clinical value of MRI in diagnosis of rotator cuff tear. Methods We retrospectively analyzed and compared the MRI and arthroseopeimages from sixty-eight patients who were diagnosed as rotator cuff tear from Jun. 2014 to Dec. 2015. Results In the 68 patients, the clinical diagnosis of 50 patients were confirmed using arthroseope, and no positive findings were found in 18 patients. In the patients with rotator cuff tear, preoperative MRI diagnosis was positive in 44 pa- tients, and negative in 6 patients. The sensitivity was 88%. In the 18 patients with normal arthroseope findings, preoperative MRI diagnosis was positive in 3 patients, and negative in 15 patients. The specificity was 83.3% , and the diagnostic accuracy was 86. 8%. Conclusion For the diagnosis of rotator cuff injury, MRI has a high clinical value. As a non-invasive technique, it is worthy of clinical application. However, there are still some misdiagnosis cases, so arthroscopy is recommended as a supplemented method if necessary.
