心踝血管指数在老年慢性心衰患者运动康复效果中的评价与应用研究

Effectiveness evaluation of exercise rehabilitation for the aged chronic heart failure patients based on cardio-ankle vascular index

目的探讨心踝血管指数在老年慢性心力衰竭患者运动康复效果评价中的应用价值。方法选取2014年5月-2015年10月我院住院治疗的老年慢性心力衰竭患者49例,行去除诱因、强心、利尿和扩血管等基础治疗,并根据病情调整药物剂量,在进行常规护理基础上,根据心功能分级实施运动康复护理,比较治疗前后的患者心踝血管指数、心率、射血分数和步行距离(6min),采用明尼苏达生活质量问卷进行生活质量评价。结果 49例患者治疗护理后的心踝血管指数、心率、射血分数、步行距离(6min)及生活质量评分平均值与治疗护理前比较差异有统计学意义(P<0.05)。结论运动康复治疗后患者左室射血分数(LVEF)与步行距离(6min)、心踝血管指数与明尼苏达生活质量评分之间变化的一致性证明心踝血管指数可以通过抗心衰治疗及运动康复训练而得以下降,故可将其作为心力衰竭治疗及运动康复评价效果的参考指标之一。

Objective To discuss the effectiveness evaluation of exercise rehabilitation in aged chronic heart failure patients based on cardio-ankle vascular index（CAVI）. Methods 49 cases of elderly hospitalized patients with chronic heart failure in our hospital from October 2015 to May 2014 were selected. They were given removing incentives, strong heart, diuretic and expansion of blood vessels and other basic treatment, and adjust the drug dosage according to the disease condition. On the basis of routine nursing, the exercise rehabilitation was carried out according to the function of the heart function. The CAVI, heart rate, ejection fraction and walking distance （6min） were compared before and after treatment. The quality of life was evaluated by using the Minnesota living with heart failure ques- tionnaire（LiHFe）. Results The CAVI, heart rate, ejection fraction, walking distance （6min） and the quality of life score were significantly different among 49 patients after treatment （P ~ 0.05）. Conclusion The changes of left ven- tricular ejection fraction （LVEF） and walking distance （6 min）, heart ankle vascular index and the quality of life in Minnesota was consistent after exercise rehabilitation. It is shown that the cardiac ankle vascular index can be de creased by anti heart failure treatment and exercise rehabilitation training. Therefore, it can be used as a reference index for the treatment of heart failure and the effect of exercise rehabilitation evaluation.
