尿毒症维持性血液透析患者血清CRP与营养状态及感染的相关性 被引量:17

Uremia Patients with Maintenance Hemodialysis: Correlation between Serum CRP and Nutritional Status and Infection

摘要 目的:探讨尿毒症维持性血液透析患者的血清C-反应蛋白(CRP)与营养状态及感染的相关关系。方法:按照是否发生感染将2013年2月-2015年8月我院收治的238例尿毒症维持性血液透析患者分为感染组(118例)和非感染组(120例),对两组患者的血清CRP、营养状态相关指标进行比较,并分析CRP水平与营养状态及感染的相关性。结果:感染组的体质量指数(BMI)、白蛋白、前白蛋白、转铁蛋白、血红蛋白水平低于非感染组,重组人促红细胞生成素、血清CRP水平高于非感染组,肱三头肌皮褶厚度低于非感染组,差异均有统计学意义(P<0.05)。经Pearson积矩相关分析,血清CRP与BMI、白蛋白、前白蛋白、转铁蛋白、血红蛋白呈负相关关系(r=-0.81、-0.93、-0.85、-0.79、-0.91),差异有统计学意义(P<0.05),血清CRP与肱三头肌皮褶厚度(TSF)呈负相关关系(r=-0.76,P<0.05),与重组人促红细胞生成素呈正相关关系(r=0.89,P<0.05)。结论:尿毒症维持性血液透析患者的血清CRP水平可作为评估患者营养状态及感染的重要指标,具有重要的参考价值。 Objective: To explore the correlation between serum C reactive protein(CRP) and nutritional status and infection in uremia patients with maintenance hemodialysis. Methods: A total of 238 uremia patients with maintenance hemodialysis, who were admitted to Langfang People’s Hospital of Hebei Province from February 2013 to August 2015, were divided into infection group(118 patients) and non-infection group(120 patients) according to whether occurred the infection. The serum CRP and relevant indexes of nutritional status of patients in the two groups were compared,and the correlation between CRP and nutritional status and infection was analyzed. Results: The BMI, albumin, prealbumin, transferrin and haemoglobin levels in infection group were lower than those in non-infection group, but the erythropoietin and serum CRP were higher than those in non-infection group,and the triceps skinfold thickfold was lower than that in non-infection group, the differences were statistically significant(P〈0.05). The Pearson product monment correlation analysis showed that there was a negative correlation between CRP and BMI, albumin, prealbumin, transferrin and haemoglobin levels(r=-0.81,-0.93,-0.85,-0.79,-0.91), the difference was statistically significant(P〈0.05). There was a negative correlation between CRP and triceps skinfold thickfold(r=-0.76, P〈0.05), and there was a positive correlation between CRP and erythropoietin(r=0.89, P〈0.05).Conclusion: The serum CRP of uremia patients with maintenance hemodialysis can be used as an important index to evaluate the nutritional status and infection, which has an important reference value.
