乳腺癌患者超声中心静脉导管引导置管感染的分析 被引量:6

Influence of ultrosound-guided catheterization on infections for breast cancer patients

摘要 目的比较乳腺癌术后化疗经超声引导中心静脉导管置管患者感染病原菌分布及预防措施,为降低置管感染率提供参考依据。方法选择医院2013年8月-2015年8月乳腺癌术后化疗患者200例作为研究对象,按随机数字表法随机分为两组,每组各100例;所有患者在手术后均接受中心静脉导管置管化疗,对照组采用传统经外周静脉置入中心静脉导管(PICC),研究组采用超声引导下行中心静脉导管置管,比较两组患者总置管成功率及病原菌分布,数据采用SPSS 19.0软件进行统计分析。结果研究组患者置管总成功率为94.0%,对照组患者置管总成功率为71.0%;研究组患者共检出6株病原菌,以金黄色葡萄球菌为主占50.0%;对照组患者共检出14株病原菌,以表皮葡萄球菌为主占50.0%,两组比较,差异均有统计学意义(P<0.05)。结论超声引导置管在乳腺癌术后化疗患者中应用,能增加置管成功率,降低感染率,应重视病原菌分布的检测,降低医院感染的发生。 OBJECTIVE To compare bacteria distribution of ultrasound-guided central venous catheter insertion infection in breast cancer patients postoperative chemotherapy,so as to provide the basis for reducing catheter infection rates.METHODS A total of 200 cases of breast cancer patients with postoperative chemotherapy from Aug.2013 to Aug.2015 in our hospital were selected and divided into two groups according to random numeration table,with 100 patients in each group.All the patients received the central venous catheter insertion chemotherapy after surgery.The study group was treated with ultrasound-guided catheterization,and the control group was given the conventional peripheral vein in central venous catheter(PICC).The total success rates of catheterization and infected bacteria distribution were observed and compared between the two groups.SPSS 19.0was used for data analysis.RESULTS The total success rate was 94%in study group,significantly higher than 71%in control group.There were 6strains of pathogens detected from study group,mainly Staphylococcus aureus,accounting for 50.0%,and 14 strains detected from control group,mainly Staphylococcus epidermidis,accounting for50.0%,the difference was significant(P〈0.05).CONCLUSIONThe one-time success rate of ultrasound-guided cathererization can be increased,and the incidence of wound infection can be reduced,by using ultrasound-guided central venous catheter insertion.The bacteria distribution is valuable for preventing infection to reduce the incidence of nosocomial infections.
