卡环式可摘义齿修复口底肿瘤术后牙列缺损的临床观察

Clinical Observations of Removable Partial Denture in Repairing Dentition Defect Caused by Mouth Floor Carcinoma Surgery

目的评价并比较卡环式可摘义齿对口底肿瘤术后牙列缺损和普通牙列缺损修复的效果。方法于2007年1月至2014年6月,选择48例行保持下颌骨连续性的口底肿瘤切除术后牙列缺损患者(观察组)和48例普通牙列缺损患者(对照组),采用卡环式可摘义齿进行牙列缺损修复。定期随访并测定2组的咀嚼效率和基牙牙周指数,并进行满意度评价。结果修复前观察组的咀嚼效率和牙龈指数均低于对照组,差异均有统计学意义(P<0.05);菌斑指数也低于对照组,但差异无统计学意义(P>0.05)。修复后不同时期观察组的咀嚼效率、牙龈指数及菌斑指数均增高,且与对照组的差异均无统计学意义(P>0.05)。修复18个月后观察组的满意度高于对照组,但差异无统计学意义(P>0.05)。结论卡环式可摘义齿能应用于口底肿瘤术后牙列缺损的修复。

Objective To evaluate and compare the effect of clasp removable partial dentures in repairing dentition de- feet caused by mouth floor carcinoma surgery and ordinary defect of dentition. Methods 48 patients with dentition defect who ac- cepted mouth floor carcinoma resection with mandibular continuation were the observation group, and 48 patients with ordinary dentition defect were the control group. Clasp removable partial dentures were used to repair dentition defect. The 2 groups of pa- tients were followed up regularly, their masticatory efficiency, butmentg periodontal index were determined, and satisfaction evalua- tion were done. Results The mastieatory efficiency, gingival index （ GI ） of the observation group were lower than those of the control group, there had statistically significant difference （ P 〈 0.05 ） before repairing. The plaque index was also lower than that of the control group, however, the difference was not statistically significant （P 〉 0.05 ）. At the different stages ＇after repairing, the masticatory efficiency, GI and PI all increased, and there had no statistically significant difference （P 〉 0.05 ）. Satisfaction of the observation group 18 months later was higher than that of the control group, but the difference was not statistically significant（P 〉 0.05）. Conclusion The clasp removable partial dentures can be used to repair the dentition defect deriving from mouth floor carcinoma surgery.
