同步放化疗与单纯放疗治疗ⅡB～ⅢB期宫颈癌的临床效果对比

Clinical Effects of Concurrent Chemoradiotherapy and Simple Radiotherapy for Patients with ⅡB～ⅢB Cervical Cancer

目的 比较同步放化疗与单纯放疗治疗ⅡB~ⅢB期宫颈癌的临床效果。方法 按照治疗方案不同将93例宫颈癌患者分为实验组（同步放化疗）50例和对照组（单纯放疗）43例,实验组患者给予同步放化疗治疗,对照组患者给予单纯放疗治疗,比较2组治疗效果及不良反应。结果 实验组患者RR率明显高于对照组,差异具有统计学意义（P〈0.05）。实验组患者胃肠道反应、骨髓抑制发生率及严重程度均明显高于对照组,差异具有统计学意义（P〈0.05）;2组患者放射性直肠炎和放射性膀胱炎发生率及严重程度比较,差异无统计学意义（P〉0.05）。18~24个月随访期间2组患者无瘤生存率、复发率、转移率、死亡率比较,差异无统计学意义（P〉0.05）。结论 同步放化疗能够显著提高近期肿瘤治疗效果,但会明显增加胃肠道反应和骨髓抑制的发生率及严重程度,远期无瘤生存率略高于单纯放疗,但还需继续延长随访时间,进一步进行研究。

Objective To explore the clinical effects of concurrent ehemoradiotherapy and simple radiotherapy for pa- tients with ⅡB～ⅢB cervical cancer. Methods 93 cases of patients with cervical cancer were divided into the experiment group （50 cases,concurrent ehemoradiotherapy） and the control group（43 cases, simple radiotherapy） ,the experiment group were trea- ted by concurrent chemoradiotherapy, the experiment group were treated by simple radiotherapy. The clinical effects and side effect were compared. Results The RR rate of the experiment group were higher than that of the control group （ P 〈 0.05 ） ;The gastro- intestinal tract response,incidence of bone marrow suppression, severity in the experiment group were lower than those of the con- trol group（ P 〈 0.05 ） ;There had no significant difference on the rate of radiation proetitis and radiation-induced cystitis between the 2 groups （ P 〉 0.05 ） ; There had no significant difference in the rate of tmnor-free survival, relapse, lymph node metastasis, and death （ P 〉 0.05 ）. Conclusion Concurrent chemoradiotherapy for cervical cancer can increase clinical effects,it can increase the gastro-intestinal tract response and myelosuppression, the long-term disease-free survival is higher than simple radiotherapy. It need to prolong follow-up for further studies.
