12例新型隐球菌肺炎诊治临床分析

Clinical Analysis of Diagnosis and Treatment of 12 Cases of Pulmonary Cryptococcosis

目的:通过对新型隐球菌肺炎诊治的临床分析探讨该疾病的早期诊治要点,从而进一步提高临床医生对该病的认识。方法:收集2014年1月-2016年1月笔者所在医院收治的12例隐球菌肺炎患者的临床资料,对患者的诊治经过、相关辅助检查、病理结果等进行回顾性分析。结果:12例患者中10例患者经氟康唑和/或氟胞嘧啶等药物治疗,复查肺部CT提示明显吸收好转,1例患者放弃治疗自动出院,另有1例患者失访。结论:隐球菌肺炎无明显临床特异性,早期完善相关辅助检查是诊断治疗的关键,同时需进一步提高医务工作者对该病的认识,降低误诊率。

Objective： To explore the key points of early diagnosis and treatment of pulmonary cryptococcosis through the clinical analysis, improving the clinician＇s understanding of this disease.Method： The clinical data of 12 cases of patients diagnosed as pulmonary cryptococeosis in our hospital from January 2014 to January 2016 were collected, and the patient＇s clinical characteristic, diagnosis and treatment, the relevant auxiliary examinations, pathological results were retrospectively analyzed.Result： The CT review showed significantly improved and absorption in 10 of the 12 patients after treated by Fluconazole and/or Flucytosine, 1 patient gave up the treatment and discharged from hospital, 1 patient was lost to follow-up.Conclusion： There is no obvious clinical specificity in the cryptococcal pneumonia patients.The key to the diagnosis and treatment is to improve the relevant auxiliary examination.At the same time, it is necessary to improve the cognition of the disease in the medical practitioners and reduce the misdiagnosis rate.
