2型糖尿病患者下肢动脉病变的相关因素分析

Related factors for peripheral artery disease in patients with type 2 diabetes

目的探讨影响T2DM患者下肢动脉病变(PAD)的相关因素。方法采用多普勒超声测定520例T2DM患者上臂和踝部的SBP,计算踝肱指数(ABI),根据ABI,将研究对象分为PAD组(ABI≤0.9)和非PAD组(ABI>0.9),比较两组年龄、性别、吸烟指数、血脂、HbA_1c和病程等指标,并分析T2DM患者PAD的危险因素。结果 520例T2DM患者中,合并PAD者105例,发生率为20.2%。与非PAD组比较,PAD组年龄、吸烟指数、病程、高血压患病率、SBP、DBP、血清LDL-C、LDL-C/HDL-C、TC/HDL-C均升高(P<0.01)。Logistic回归分析显示,T2DM患者PAD的独立危险因素为年龄和吸烟指数(OR=1.077、1.021)。结论增龄和吸烟指数是T2DM患者发生PAD的独立危险因素。病程长、高血压和脂代谢异常与PAD发生有关。

Objective To investigate related factors for peripheral artery disease （PAD） in patients with type 2 diabetes mellitus. Methods A total of 520 patients with type 2 diabetes mellitus （T2DM） were enrolled in this study. SBP levels of the upper arm and ankle were tested and the ankle brachial index （ABI） was calculated after that. All the participants were divided into PAD group （ABI ≤ 0.9） and non PAD group （ABI〉0. 9） according to their ABI. Patients＂ age,gender,smoking index,blood lipid, HbA1 c, duration and so on were compared between two groups and the risk factors for PAD were evaluated. Results There were 105 patients with PAD in all the 520 subjects, consisting 20.2%. Age, smoking index, duration of the disease, prevalence of hypertension, SBP, DBP, serum LDL-C, low density lipoprotein cholesteroi to high-density lipoprotein cholesterol ratio （LDL-C/HDL-C）,total cholesterol to high-density lipoprotein cholesterol ratio （TC/HDL - C） were all increased in PAD group than in non PAD group （P〈 0. 01）. Logistic regression analysis showed that age and smoking index were independent risk factors for PAD in patients with T2DM （OR= 1. 077,1. 021）. Conclusion Age and smoking index are independent risk factors for PAD in patients with T2DM. Long duration of the disease, hypertension and lipid metabolism disorders are related to the occurrence of PAD.
