加强药师合理用药的处方干预及典型处方分析

More Pharmacists' Active Intervention on Rational Drug Use and Analysis of Typical Outpatient Rrescription

目的探讨门诊处方常见问题,建立药师在门诊合理用药中主动干预的机制,促进门诊合理用药。方法自2013年我院开始采用药师在调剂中主动干预、对问题处方在全院点评通报、行政和经济处罚等措施,对门诊的不规范处方、用药不适宜处方、超常处方进行干预。结果 2014年6月随机抽取2012年7~12月与2013年7~12月的每月1500张门诊处方各计9000张,进行分析,结果门诊不合理用药和超常处方数量减少。结论通过干预,门诊处方的合理性提高,药师对处方合理化干预有效,提高了药物治疗水平。

Objective The present study here was carried out to investigate the common problems of outpatient prescriptions and establish system of pharmacists＇ active intervention on rational drug use to improve the rational use of drugs. Methods From 2013, some measures including the pharmacists＇ active intervention for irrational use of drugs, comments on unqualified prescriptions conducted in the hospital and administrative and economic punishments, were proposed to reinforce intervention on non-standard outpatient prescriptions, inappropriate prescriptions and extraordinary prescriptions. Results Every month, 1500 outpatient prescriptions were randomly selected between July 2012 and December 2012 as research data. In the same way, 9000 outpatient prescriptions from July 2013 to December 2013 were also randomly selected. Analysis were conducted and observed that there was a decrease in the number of irrational prescriptions and extraordinary prescriptions. Conclusions The intervention of pharmacists was proved to be very effective in rational drug usage, which played an important role in improving the levels of drug therapy.
