肾脏嗜酸细胞腺瘤4例临床病理分析附文献复习

Clinical and Pathological Analysis of 4 Cases of Renal Cell Carcinoma of the Kidney: a Review of Literature

目的探讨肾脏嗜酸细胞腺瘤的临床病理特点和免疫组化特征及鉴别诊断。方法收集2005年~2014年我院4例肾脏嗜酸细胞腺瘤的临床资料,分析其临床病理特点及免疫表型,并复习相关文献。结果 4例患者均缺乏特异的临床特征,中位年龄65岁,平均随访时间32个月,均健在。CT增强扫描后瘤体中央可见星状低密度区是该瘤的主要特征。结论肾嗜酸细胞腺瘤是一种少见的良性肾上皮性肿瘤,巢团状排列的瘤细胞分布于细胞稀少、疏松水肿的纤维结缔组织中,肿瘤细胞EMA、CD117、CK18（＋）,而vimentin和CD10（-）,常易被误诊为其他胞质嗜酸性肾上皮性肿瘤。掌握其临床影像和病理形态特征及免疫表型特点是正确诊断及鉴别诊断的关键。

Objective To investigate the renal oxyphil cell adenoma clinicopathologic features and immunohistochemical characteristics and differential diagnosis. Methods From 2005 to 2014 Years in our hospital, 4 cases of renal oxyphil cell adenoma of the clinical data, analyze the clinical pathological characteristics and immunophenotype and review the relevant literature. Results Four patients were lack of specific clinical features, the median age was 65 years old. The average follow-up time was 32 months, were alive. CT enhanced scan the tumor can be seen in the central star shaped low density area is the tumor is the main feature. Conclusion Renal oxyphil cell adenoma is a rare benign renal epithelial neoplasms, nest like arrangement of tumor cell distribution in fine rare cells, edema and looseness of fibrous connective tissue and tumor cell EMA, CD_（117）, CK18（＋）, and vimentin and CD_（10）（-）, often easy was misdiagnosed as other cytoplasmic eosinophilia acid renal epithelial neoplasms. Master the imaging and clinical pathological features and immunophenotypic features of is the key to the correct diagnosis and differential diagnosis.
