我院门诊不合理用药处方分析

Analysis of irrational prescriptions in outpatient clinic of our hospital

目的：提高处方质量,促进合理用药。方法：随机抽取我院2013年1月~2014年12月门诊处方4745张,对不合理用药处方进行分析。结果：4745张处方共筛选出不合理用药处方431张,其中,联合用药不合理227张,占比52.67%;药物用法、用量不合理96张,占比22.27%;选药不当68张,占比15.18%;溶媒选择不当25张,占比5.80%;用药与诊断不符15张,占比4.08%。结论：开展用药处方点评,对不合理用药处方进行分析,既可提高药学人员的业务能力,又能将信息反馈给临床医生,保证药物的疗效,减少不良反应,促进合理用药。

Objective： to improve the quality of prescriptions and to promote the rational use of drugs. Methods： randomly selected from our hospital in January 2013 ~2014 December outpatient prescription 4745, the irrational use of prescription analysis. Results： 4745 prescriptions were screened out unreasonable prescriptions 431 Zhang, the combined medication unreasonable 227, accounted for52.67%; drug usage, dosage unreasonable 96, proportion of 22.27%; improper drug selection 68 pieces, accounted for 15.18%;inappropriate choice of solvent 25, proportion of 5.80%; medicine and diagnosis inconsistent 15, proportion of 4.08%. Conclusion： to carry out the prescription comment, the irrational prescriptions were analyzed, can improve the operational capacity of the pharmacy staff,and feedback the information to clinicians, ensure drug curative effect, reduce adverse reactions, to promote the rational use of drugs.
