微创撬拔联合空心针固定治疗跟骨非粉碎性骨折的可行性研究

Feasibility study on minimally invasive pry pull reset combined with hollow needle fixed for heel bone comminuted fracture treatment

目的对微创撬拔联合空心针固定治疗跟骨非粉碎性骨折的疗效及安全性等予以研究,证实其可行性。方法选取在我院进行手术治疗的158例跟骨骨折患者为研究对象,随机分为观察组(80例)和对照组(78例);观察组行微创撬拔联合空心针固定治疗,对照组行切开复位联合加压螺丝钉内固定治疗。术后比较两组手术成功率、术中指标、安全性、随访6个月患肢功能评分等。结果观察组手术成功率为97.5%,对照组手术成功率为100.0%,两组手术成功率差异无统计学意义(P>0.05)。创口长度、出血量、手术时间及VAS评分等术中数据比较,观察组均显著优于对照组(P<0.05);观察组并发症发生率显著低于对照组(P<0.05);随访6个月观察组患肢功能优良率为77.5%,略低于对照组的80.8%(P>0.05)。结论微创撬拔联合空心针固定治疗跟骨非粉碎性骨折,具有成功率高、创伤小、出血少、手术时间短、疼痛度低、安全性高及预后良好等优势,具有可行性和推广价值。

Objective To verify the feasibility, clinical efficacy and safety of minimally invasive pry pull reset combined with hollow needle fixed for heel bone comminuted fracture treatment. Methods One hundred and fitty-eight cases of patients with calcaneal fracture were collected in our hospital, and were divided into observation group（80 cases） and control group（78 cases）. The patients painters in observation group were treated with minimally invasive joint hollow needle, on the other hand the patients in control group were treated with internal compression screw fixation by cutting the bone joint. Datas concerning the surgical success rate, intraoperative data, safety level, 6-month follow-up study on limb function after surgery, etc. were collected and compared. Results The success rate of the observation group was 97.5%, and in the control group was 100.0%, there was no significant difference（P〉0.05） in the two surgical success rate. Comparison of the wound length, amount of bleeding, surgical time and VAS scores, show that the observation group was significantly better than that in the control group（P〈0.05）; the complications rate in the observation group was significantly lower than that in the control group（P〈0.05）; 6-month follow-up study, excellent rate of limb function in the observation group was77.5%, which was slightly lower than that in the control group of 80.8%（P〉0.05）. Conclusion The method of minimally invasive pry pull reset combined with hollow needle fixed for heel bone comminuted fracture treatment has the advantage of high success rate, less trauma, less bleeding, shorter operative time, low degree of pain, high safety level, good healing result etc, which was feasibility and worth promotion.
