摘要
目的探讨综合管理模式对预防婴幼儿毛细支气管炎后喘息的影响。方法选取2013年1-10月在该院住院并确诊为毛细支气管炎的93例患儿并进行回顾性分析,将痊愈出院后1年内持续接受综合管理的54例患儿纳入观察组,没有持续接受综合管理的39例患儿纳入对照组。综合管理包括建立电子档案、实施健康教育、电话追踪随访、居家管理等。统计两组喘息发生次数和例数,出院满1年行肺功能检测,并对家属进行相关健康知识问卷调查。结果出院后1年内观察组发生喘息率为25.92%(14/54),对照组发生喘息率为46.15%(18/39),差异具有统计学意义(P<0.05);出院后1年内两组肺功能各项指标均较出院时明显好转,差异具有统计学意义(P<0.05);家属对相关健康知识掌握率达85%以上。结论对毛细支气管炎后婴幼儿进行综合管理,可以提高治疗的依从性,减少喘息的发生。
Objective To explore the effect of integrated management model on prevention of wheezing after capillary bronchitis among infants. Methods Ninety- three hospitalized infants diagnosed as capillary bronchitis from January to October in 2013 were selected and analyzed retrospectively,then the infants were divided into observation group( 54 infants received comprehensive management continously within one year after leaving the hospital) and control group( 39 infants didn't receive comprehensive management continously). Comprehensive management included establishing electronic records,conducting health education,telephone survey,daily life management,and so on. The times and numbers of wheezing were recorded,pulmonary function examination was conducted after one year of discharge,the family members were surveyed by a questinnaire about related health knowledge. Results The incidence rates of wheezing within one year after discharge in observation group and control group were 25. 92%( 14 /54) and 46. 15%( 18 /39),respectively,there was statistically significant difference( P〈0. 05); pulmonary function in the two groups was improved within one year after discharge comapred with at discharge,there was statistically significant difference( P〈0. 05); family members mastered more than 85% of relevant health knowledge. Conclusion Integrated management of infants with wheezing after capillary bronchitis can improve the treatment compliance and reduce the incidence rate of wheezing.
