HPVE6/E7mRNA在宫颈病变中的应用价值 被引量:9

摘要 目的对宫颈病变患者宫颈HPVE6/E7mRNA的表达情况进行检测,探讨HPVE6/E7mRNA在宫颈病变诊断中的应用价值。方法选择2014年12月-2016年4月在金华市浦江县妇幼保健院宫颈门诊进行宫颈液基细胞血检查及宫颈病理学检查的500例患者作为研究对象,其中病理检查结果显示宫颈正常或者宫颈慢性炎症者314例,宫颈上皮内瘤变(CIN)Ⅰ级者78例,CINⅡ级39例,CINⅢ级者33例,宫颈癌16例。观察宫颈不同病变组患者中HPV DNA和HPVE6/E7mRNA的检测情况、宫颈HPV DNA检测结果及HPVE6/E7mRNA检测结果和宫颈病理结果的比较情况、HPV DNA和HPVE6/E7mRNA对宫颈癌及宫颈CIN的筛查结果比较情况。结果宫颈不同病变组患者HPV DNA阳性率比较,差异有统计学意义(P<0.05)。宫颈HPV DNA检测结果和宫颈病理结果比较,差异有统计学意义(P<0.05)。宫颈不同病变组患者HPVE6/E7mRNA阳性率比较,差异有统计学意义(P<0.05)。宫颈HPVE6/E7mRNA检测结果和宫颈病理结果比较,差异有统计学意义(P<0.05)。HPVE6/E7mRNA对宫颈癌和宫颈CIN的筛查特异度和阳性预测值均高于HPV DNA(P<0.05)。结论 HPVE6/E7mRNA对宫颈疾病的预测灵敏度和HPV DNA相似,但HPVE6/E7mRNA的预测特异度高于HPV DNA,在宫颈病变的筛查中具有一定意义,可以作为宫颈病变的筛查方法。
