神经白塞氏病13例临床分析

A clinical analysis of Neuro-Behcet's disease in 13 patients

目的:探讨神经白塞氏病(NBD)的临床特征及诊治方法。方法:收集本院风湿科从2005年至2014年收治的13例NBD患者,回顾性分析患者的临床资料。结果:NBD临床表现包括头痛(9例),发热(7例),精神症状(4例),肢体瘫痪、麻木(3例),共济失调(2例),构音障碍(1例)及眩晕(1例)。头颅MRI提示病变部位主要分布在中脑(5例)、脑桥(2例)及基底节区(2例)。患者均给予糖皮质激素免和疫抑制剂后症状缓解。结论:NBD预后较差,为白塞氏病的重症表现之一。了解NBD的临床特征,有利于对该类患者进行及早的诊断与治疗。

Objective：To summarize on the clinical features, diagnosis and treatment of Neuro-Behcet′s disease （ NBD） . Methods：A retrospective analysis was performed on clinical data from 13 NBD patients treated in our Department of Rheumatology and Immunology between 2005 and 2014. Results：Clinical manifestations of NBD included headache （n=9）,fever （n=7）,mental disorders （n=4）,paralysis or numbness of limbs （n=3） ,ataxia （ n=2） ,dysarthria （ n=1） and vertigo （ n=1） . Cranial MRI showed that the lesions mainly involved the mid-brain （ n=5） ,pons （ n=2） and basal ganglia （ n=2） . All patients were symptomatically relieved after treated with corticosteroids and immunosuppressants. Conclusion：NBD is poor in prognosis, and represents a severe form of Behcet′s disease. Understanding the clinical features of NBD may help early diagnosis and treatment of these patients.
