摘要
目的:观察腰椎爆裂性骨折患者使用脊柱前路内固定系统和脊柱后路锥弓根钉棒系统治疗的临床疗效。方法:选取2014年2月-2015年2月本院收治的腰椎爆裂性骨折患者80例作为研究对象,随机分为前路组和后路组,每组40例。前路组给予脊柱前路内固定系统治疗,后路组给予脊柱后路锥弓根钉棒系统治疗,观察两组患者手术时间、术中出血量、术后引流量、融合时间、Cobb角变化、椎管前后径及椎管横断面积情况。结果:前路组手术时间、术中出血量及术后引流量均高于后路组,比较差异均有统计学意义(P<0.05);两组术前Cobb角、椎管前后径及椎管横断面积比较差异均无统计学意义(P>0.05),术后1年前路组均低于后路组,比较差异均有统计学意义(P<0.05)。结论:两种治疗方法治疗腰椎爆裂性骨折的疗效都较好,脊柱前路内固定器手术较为复杂,脊柱后路椎弓根钉棒系统术后cobb角损失较大,故应该根据患者的具体情况选择合适的治疗方式。
Objective:To observe the clinical effects of anterior spinal internal fixation device system and posterior pedicle screw rod system in treatment with lumbar burst fracture.Method:From February 2014 to February 2015,80 cases of lumbar burst fracture in our hospital were selected as the research objects,they were randomly divided into the anterior group and the posterior group,each group had 40 cases.The anterior group were treated with anterior spinal internal fixation device system and the posterior group were treated with posterior pedicle screw rod system,the operation time,intraoperative blood loss,postoperative drainage volume,fusion time,Cobb angle,anterior and posterior spinal canal diameter,cross-sectional area of spinal canal of two groups were compared.Result:The operation time,intraoperative blood loss and postoperative drainage volume in the anterior group were lower than the posterior group,the differences were statistically significant(P〈0.05).There were no significant differences in Cobb angle,anterior and posterior spinal canal diameter and cross-sectional area of spinal canal in two groups(P〉0.05),after 1 year,the anterior group was lower than the posterior group,the differences were statistically significant(P〈0.05).Conclusion:The therapeutic effect of two treatment methods in the treatment with lumbar burst fracture is better,anterior spinal internal fixation device system surgery more complex,posterior pedicle screw rod system postoperative Cobb angle loss is larger,therefore,we should choose the appropriate treatment according to the patient's specific conditions.
